Point-of-care testing (POCT) at ED triage prior to physician evaluation may help identify critically ill patients. We studied the impact of ED POCT in a single ED with a high degree of crowding for patients with high-risk complaints who were triaged as non-critically ill. \nMethods: \nWe conducted the study from April–July 2017 at King Abdulaziz University (KAU) Hospital in Jeddah, Saudi Arabia. Patients with one of seven complaints received triage POCT. The primary outcome was whether POCT results at triage resulted in immediate transfer of the patient from the waiting room into the ED. Secondary outcomes were whether the triage nurse felt that the POCT results were useful, and whether triage POCT changed triage acuity. We used simple descriptive statistics to summarize the data. \nResults:\n A total of 94 patients were enrolled and received i-STAT® POCT. The most common symptoms and triage protocols were for chest pain (42%), abdominal pain (31%), and shortness of breath (22%). In 11 cases (12%), care was changed as a result of triage POCT. In 12 cases (13%), triage level was changed. The triage nurse found POCT helpful in 93% of cases.\nConclusion:\n In this ED, triage POCT was a helpful adjunct at ED triage and resulted in immediate care (transfer to an ED room) in one in eight cases. Given this increasing competition for emergency medicine (EM) residency positions, understanding factors that contribute to match success is important to ensure a successful match for osteopathic medical students. \nMethods:\n Our anonymous survey to evaluate factors that led to a successful match was sent out to residents in current ACGME-, AOA-, and dually-accredited programs via the AOA program director listserv and the Council of Residency Directors (CORD) e-mail listserv in 2017. \nResults:\n We had 218 responses. Responses showed that osteopathic graduates had less affiliation with EM residencies, their home institutions provided less information regarding standardized letters of evaluations (SLOE), and that successful osteopathic graduates seemed to learn about them while on EM elective rotations. We used a two-level hierarchical logistic regression model to estimate inter-physician variation. Intraclass correlation coefficient (ICC) was calculated. \nResults:\n The hierarchical logistic regression analyses showed that patient-visit factors including younger age, arrival mode by ambulance, prior CT, >79 ED arrivals in the previous four hours, and ultrasound had statistically significant negative associations with physician CT ordering, while surgical team admission and white blood count (WBC) >12.5 K/millimeter cubed (mm3) had statistically significant positive associations with physician CT ordering. With physician-level factors, only physicians with >21 years experience after medical school graduation showed statistical significance negatively associated with physician CT ordering. Our data demonstrated increased CT ordering from the mean in only one out of 43 providers (2.3%), which indicated limited variation across physicians to order CT. Variables collected included length of program, curriculum (e.g., clinical rotations, didactic experience, and research opportunities), size of program/number of trainees, affiliation with emergency medicine (EM) residency, geographic location, and salary.\n \nResults:\n We identified 29 EMPA postgraduate training programs in 17 states, with at least one additional program in development. The mean length of EMPA training programs is 15 months (range 12-24 months). The most common non-ED/elective rotations are orthopedics, ultrasound, anesthesiology, and trauma. The mean number of trainees per class is 3.46 (median 3, range 1-16 trainees); 27 of 29 (93%) programs were in institutions that also had an EM residency program. The procedure involves placing a single-lumen catheter in the IJ vein under ultrasound (US) guidance. As this technique is relatively new, the details regarding the ease of the procedure, how exactly it should be performed, and the safety of the procedure are uncertain. Our primary objective was to determine the success rate for peripheral IJ placement. Secondarily, we evaluated the time needed to complete the procedure and assessed for complications.\nMethods:\n This was a prospective, single-center study of US-guided peripheral IJ placement using a 2.5-inch, 18-gauge catheter on a convenience sample of patients with at least two unsuccessful attempts at peripheral IV placement by nursing staff. Peripheral IJ lines were placed by emergency medicine (EM) attending physicians and EM residents who had completed at least five IJ central lines. All physicians who placed lines for the study watched a 15-minute lecture about peripheral IJ technique. A research assistant monitored each line to assess for complications until the patient was discharged.\nResults:\n We successfully placed a peripheral IJ in 34 of 35 enrolled patients (97.1%). The median number of attempts required for successful cannulation was one (interquartile range (IQR): 1 to 2). The median time to successful line placement was 3 minutes and 6 seconds (IQR: 59 seconds to 4 minutes and 14 seconds). Two lines failed after placement, and one of the 34 successfully placed peripheral IJ lines (2.9%) had a complication – a local hematoma. There were, however, no arterial punctures or pneumothoraces. It is unknown if patients with malignancies and hyperlactatemia are at increased risk of mortality. If serum lactic acid could improve detection of at-risk patients with cancer, it would be useful in risk stratification. There is also little evidence that “alarm” values of serum lactate (such as >/=4 mmol/L) are appropriate for the population of patients with cancer.\nMethods:\n This was a continuous retrospective cohort study of approximately two years (2012-2014) at a single, tertiary hospital ED; 5,440 patients had serum lactic acid measurements performed in the ED. Of the 5,440 patients in whom lactate was drawn, 1,837 were cancer patients, and 3,603 were non-cancer patients.Cumulative unadjusted mortality (determined by hospital records and an external death tracking system) was recorded at one day, three days, seven days, and 30 days. We used logistic regression to examine the risk of mortality 30 days after the ED visit after adjusting for confounders. \nResults:\n In an unadjusted analysis, we found no statistically significant difference in the mortality of cancer vs. non-cancer patients at one day and three days. Significant differences in mortality were found at seven days (at lactate levels of <2 and 4+) and at 30 days (at all lactate levels) based on cancer status. After adjusting for age, gender, and acuity level, 30-day mortality rates were significantly higher at all levels of lactic acid (<2, 2-4, 4+) for patients with malignancy. \nConclusion:\n When compared with non-cancer patients, cancer patients with elevated ED lactic acid levels had an increased risk of mortality at virtually all levels and time intervals we measured, although these differences only reached statistical significance in later time intervals (Day 7 and Day 30). Our results suggest that previous work in which lactate “cutoffs” are used to risk-stratify patients with respect to outcomes may be insufficiently sensitive for patients with cancer. Examining a cohort of adult blunt trauma patients that received paired abdominal and pelvic (A/P) CT, we sought to determine the diagnostic yield of clinically significant injuries (CSI) in the following: 1) the abdomen alone; 2) the pelvis alone; 3) the lumbosacral spine alone; and 4) more than one of these anatomic regions concomitantly.\n \nMethods:\n In this retrospective study, we reviewed the imaging and hospital course of a consecutive sample of blunt trauma activation patients older than 14 years of age who received paired A/PCT during their blunt trauma assessments at an urban Level I trauma center from April through October 2014. Categorization of CSI was determined according to an a priori, expert panel-derivedclassification scheme.\nResults:\n The median age of the 689 patients who had A/P CT was 48 years old; 68.1% were male; 64.0% were admitted, and hospital mortality was 3.6%. The purpose of this study was to analyze ED visits for sexual assault and determine if alcohol consumption by the patient was noted. \nMethods:\n This study was a retrospective chart review of patients aged 18-25 presenting to an ED in a college town over a four-year period. Extracted variables included age, gender, delay in seeking care, sexual assault nurse examiner (SANE) evaluation, and alcohol consumption by the patient. For analysis of alcohol use, cases were categorized as ages < 21 and ≥ 21. \nResults:\n There were 118 patients who presented to the ED from 2012 to 2015. The mean age of the cohort was 20 years, and almost 70% of visits were among those < 21. Of those aged < 21, 74% reported alcohol consumption, in contrast to 48% of those ≥ 21 (p = 0.055). Of those reporting alcohol use, 36% were evaluated on the day of the assault compared to 61% of those not reporting alcohol (p=0.035). \nConclusion:\n This study found that ED visits for sexual assault in emerging adults were more common in younger patients. A 10-15 feet (ft) fall is traditionally cited as an example of a high-risk mechanism, triggering trauma team activations and costly work-ups. The height and other details of mechanism are usually reported by lay bystanders or prehospital personnel. This small observational study was designed to evaluate how accurate or inaccurate height estimation may be among typical bystanders.\nMethods:\n This was a blinded, prospective study conducted on the grounds of a community hospital. Four panels with lines corresponding to varying heights from 1-25 ft were hung within a building structure that did not have stories or other possibly confounding factors by which to judge height. The participants were asked to estimate the height of each line using a multiple-choice survey-style ballot. Participants were adult volunteers composed of various hospital and non-hospital affiliated persons, of varying ages and genders. In total, there were 96 respondents. \nResults:\n For heights equal to or greater than 15 ft, less than 50% of participants of each job description were able to correctly identify the height. When arranged into a scatter plot, as height increased, the likelihood to underestimate the correct height was evident, having a strong correlation coefficient (R=+0.926) with a statistically significant p value = <0.001.\nConclusion:\n The use of vertical height as a predictor of injury severity is part of current practice in trauma triage. Rather than rely on legal frameworks for decision-making around access and use, this paper argues that adopting consent-based models of self-determination would shift the paradigm of archival policies and practices around use and access from one based on individual property rights to one based on relationships, autonomy, and prioritization of record creators and subjects. To deconstruct these systems of privilege and actively bring forth the voices of non-homogeneous archival subjects will require not just advocating for small reforms, but for entirely new ways of thinking about and doing our work. Drive-bys, violence at home, gang warfare, drug and alcohol experimentation, jail time, deaths in the family, etc., are experiences that my students face throughout their lifetimes; their lives are “rough all over.” When my students walk through the threshold of my classroom, they not only carry the weight of these experiences with them, but they also reproduce negative and destructive interactions with one another because they haven’t often experienced a world of safety and empathy. Drawing from Social Emotional Learning, CPTSD research, and Dutro’s Writing Wounded, I proposed that if students could write to one another in an anonymous journal for an extended period of time, they would be able to feel relief from expressing their traumas--starting the processing cycle--and that they could also learn to support one another empathetically. This would enable students to participate in and create a safer learning environment. Additionally, I hoped it would grow students socially and emotionally through trusting and supporting one another. I was astounded to find dozens upon dozens of empathetic, caring, and open journal entries. Students also self-reported positive feelings both toward journaling and toward the empathy they were practicing. Our understanding of the vertebrate assemblage from these strata largely comes from a single site, the Post Quarry (lower Cooper Canyon Formation), with previous research documenting a variety of temnospondyls, sphenodontians, non-archosauriform archosauromorphs, and archosauriforms including a phytosaur, three species of aetosaurs, a poposauroid, a rauisuchid, a crocodylomorph, and several dinosauromorphs. To more completely reconstruct the vertebrate assemblage of the Dockum Group of Garza County we use an apomorphy-based approach to identify morphologically similar disarticulated and fragmentary elements from a variety of localities that span the entire Cooper Canyon Formation (Norian-Rhaetian), allowing assignments from the large clade level to the species level. Many skeletal elements are incomplete yet diagnostic and are assigned to the least inclusive clade if discrete character states do not allow for an unambiguous species-level identification. We identify new specimens referable to numerous clades including Tanystropheidae, Allokotosauria + \nProlacerta\n + Archosauriformes, \nVancleavea\n + \nLitorosuchus\n, Phytosauria, Paracrocodylomorpha, Dinosauriformes, and Saurischia, in addition to additional species identifications of the aetosaur \nScutarx deltatlyus\n, and the dinosauromorph \nDromomeron gregorii\n. Our study of this material demonstrates the utility of an apomorphy-based approach in making testable and repeatable observations for identifying small, isolated fragmentary fossil tetrapod material to reconstruct a more accurate faunal hypothesis for a portion of the Late Triassic of Texas. Previous claims of the earliest dinosaurs from near the base of the Dockum Group do not pass the apomorphy-based identification test, and the question of whether the oldest known North American dinosaurs are present in the Chinle Formation or Dockum Group can be resolved by utilizing vertebrate biostratigraphic correlation. Kʷak̓ʷala has two types of objects, instrumental (=s) and accusative (=x̌). To account for their distribution, I develop a semantic theory of object case that is grounded in event structure. The first central claim of this theory is that instrumental case marks internal arguments which participate in initiating subevents (Co-initiators), while accusative case marks internal arguments which participate in non-initiating subevents (Non-initiators). Concomitantly, any internal argument which participates in both the initiating and non-initiating subevents of an event can undergo instrumental/accusative case alternation. The second central claim of this theory is that instrumental case adds semantic value, while accusative case is a meaningless default.\n Supporting evidence for these claims comes from field data. On the one hand, object case realization is constrained by verb meaning, as shown by the existence of correlations between particular semantic verb classes and particular case frames. On the other hand, evidence that case realization is determined by event structure comes from data showing that modifying event structure affects case realization. Three types of event structure modification which license case alternation include the Direct Manipulation Alternation, the Caused Motion Alternation, and semantic incorporation with the affixal verb -(g)ila ‘make’. The event-structural basis of object case is also revealed in the vicinity of weak verbs (Ritter & Rosen 1996) where the semantic value of object case is communicated independently of lexical entailments.\n This analysis allows us to see how Kʷak̓ʷala’s object case system manifests a wider cross-linguistic tendency for languages to grammaticalize a link between object-encoding and event structure. I illustrate this by showing that Kʷak̓ʷala’s object case system is semantically the mirror image of the object case system in Finnish, in which the final bound of events is grammaticalized as an interpretable accusative case (Leino 1982, Heinämäki 1984, 1994, Kratzer 2004). Taking an even wider view, Kʷak̓ʷala fits squarely within the event-structural typology proposed in Ritter & Rosen (2000), where languages are divided according to whether they grammaticalize the initial or final bound of events. We sought to determine if emergency physicians (EP) could recognize this finding in patients with suspected flexor tenosynovitis using point-of-care ultrasound (POCUS).\nMethods:\n We present a retrospective case series of seven patients suspected of PFT who had hypoechoic fluid surrounding the tendon on POCUS performed by the treating EP. We report on the patient characteristics, history of trauma by puncture wound, number of Kanavel’s signs, treatment course, and operative findings.\nResults:\n We identified seven patients whom the EP had suspected of having flexor tenosynovitis and who were subsequently found to have anechoic or hypoechoic fluid surrounding the flexor tendon on real-time POCUS examination. Patients ranged in age from 16 – 51 years. All were male. All patients had at least two of Kanavel’s signs on examination. Five of seven (71%) patients had history of recent trauma to the affected hand. Four of seven (57%) were managed in the operating room. Using game theoretic and network science methods, we found that groups would develop and maintain norms that restrict their members to join other groups. If every group can maintain such norms against every other group (the topology of the group-network is complete), the society is composed of closed communities which do not cooperate with each other. Changing the topology of the group-network can yield larger cooperating components within the population, because, in this case, members of antagonistic groups can join a third group, thereby allowing cooperation between them. On the other hand, we detect the presence of a very important exception. We show that the relationship between per capita GDP and the intensity of coups and coup attempts is not curvilinear; in this case we are rather dealing with a pronounced negative correlation; a particularly strong negative correlation is observed between this index and the logarithm of GDP per capita. We demonstrate that this fact makes the abovementioned bell-shaped relationship with respect to the integral index of sociopolitical destabilization considerably less distinct and makes a very significant contribution to the formation of its asymmetry (when the negative correlation between per capita GDP and sociopolitical destabilization among the richer countries looks much stronger than the positive correlation among poorer countries). However, our analysis shows that for all the other indices of sociopolitical destabilization we do witness the bell-shaped relationship. The case study is the frequency of entries in the \nAnnals of Ulster\n, a primary early medieval source from Ireland, which was compared to the frequency of archaeological material from early medieval Ireland. The two datasets were found to contain similar trends, namely a rapid increase in activity in the 7th Century, followed by a decline in the Early 9th Century, low levels of activity in the 10th Century, until recovery in the Late 10th / Early 11th Centuries. This overall pattern of activity had not been noticed before. Turning to the archaeological record of Britain, although there are certain similarities between Ireland and Scotland especially in the early part of the period, we find that the 9th and 10th centuries there were a stable period, and thus contrast with Ireland. We argue that environmental pressures are unlikely to be driving the signal, and instead various socio-cultural factors in the past coalesced in Ireland, leading to circumstances powerful enough to attenuate the enduring evidence for human activity, but expressing themselves silently, perhaps even in a way that was not immediately obvious to those witnessing them in the past. The simplest explanation, we contend, is that population levels fell throughout the period. However, the Northwestern (“Old Kuki”) subgroup, generally considered to be conservative, is not represented in this work as reliable data have not been available. The present study provides a comprehensive documentation of the historical phonology of one Northwestern language, Monsang. The unexpected finding is that Monsang cannot be considered conservative in its phonological development. A large number of sound changes have occurred across all phonological domains. The majority of sound changes are mergers, and with small exceptions, no unusual sound changes are found.
