Trainees’ ability to evaluate these resources is poor, and no quality assessment aids have been developed to assist them. This study derived a quality evaluation instrument for this purpose.\n \n \n \nMethods:\n We used a three-phase methodology. In Phase 1, a previously derived list of 151 OER quality indicators was reduced to 13 items using data from published consensus-building studies (of medical educators, expert podcasters, and expert bloggers) and further evaluation by our team. In Phase 2, these 13 items were converted to seven-point Likert scales used by trainee raters (n=40) to evaluate 39 OERs. The reliability and usability of these 13 rating items was determined using responses from trainee raters, and top items were used to create two OER quality evaluation instruments. In Phase 3, these instruments were compared to an external certification process (the ALiEM AIR certification) and the gestalt evaluation of 39 blog posts by 20 faculty educators.\n \n \n \nResults:\n Two quality-evaluation instruments were derived with fair inter-rater reliability: the METRIQ-8 Score (Inter class correlation coefficient [ICC]=0.30, p<0.001) and the METRIQ-5 Score (ICC=0.22, p<0.001). Both scores, when calculated using the derivation data, correlated with educator gestalt (Pearson’s r=0.35, p=0.03 and r=0.41, p<0.01, respectively) and were related to increased odds of receiving an ALiEM AIR certification (Odds Ratio=1.28, p=0.03; OR=1.5, p=0.004, respectively).\n \n \n \nConclusion:\n Two novel scoring instruments with adequate psychometric properties were derived to assist trainees in evaluating OER quality and correlated favourably with gestalt ratings of online educational resources by faculty educators. We queried our hospital laboratory database system (SunQuest®) for potassium results for specimens obtained between January 2014 and July 2014. From facility maintenance records, we identified periods of system downtime, during which specimens were hand-carried to the laboratory.\n \nResults:\n During the study period, 15,851 blood specimens were transported via our pneumatic tube system and 92 samples were hand delivered. The proportions of hemolyzed specimens in the two groups were not significantly different (13.6% vs. 13.1% [p=0.9]). Results were consistent when the criterion was limited to gross (3.3% vs 3.3% [p=0.99]) or mild (10.3% vs 9.8% [p=0.88]) hemolysis. The hemolysis rate showed minimal variation during the study period (12.6%–14.6%).\n Conclusion:\n We found no statistical difference in the percentages of hemolyzed specimens transported by a pneumatic tube system or hand delivered to the laboratory. Certain features of pneumatic tube systems might contribute to hemolysis (e.g., speed, distance, packing material). Since each system is unique in design, we encourage medical facilities to consider whether their method of transport might contribute to hemolysis in samples obtained in the emergency department. If documentation was incomplete, the study team contacted the paramedic. Paramedics use the current National Association of EMS Physicians definition of an ETI attempt (laryngoscope blade entering the mouth). We analyzed patient and EMS factors affecting ETI.\n \nResults:\n During 12,527 emergent ambulance responses, 200 intubation attempts were made in 150 patients. Intubation was successful in 113 (75%). A crew with paramedics was more than three times as likely to achieve successful intubation as a paramedic/emergency medical technician-Basic crew (odds ratio [OR], 3.30; \nP\n=.03). A small tube (£7.0 inches) was associated with a more than 4-fold increased likelihood of successful ETI compared with a large tube (³7.5 inches) (OR, 4.25; \nP\n=.01). After adjustment for these features, compared with little or no view of the glottis, a partial or entire view of the glottis was associated with a nearly 13-fold (OR, 12.98; \nP\n=.001) and a nearly 40-fold (OR, 39.78; \nP\n<.001) increased likelihood of successful intubation, respectively.\n \nConclusion:\n Successful ETI was more likely to be accomplished when a paramedic was partnered with another paramedic, when some or all of the glottis was visible and when a smaller endotracheal tube was used. However, little is known about the resuscitation of critically ill patients prior to emergency endotracheal intubation (EETI).\n \nOur aim was to describe the resuscitation practices of EM and CCM physicians prior to EETI.\n \n \n \nMethods: \nA cross-sectional survey was developed and tested for content validity and retest reliability by members of the Canadian Critical Care Trials Group. The questionnaire was distributed to all EM and CCM physician members of three national organizations. Using three clinical scenarios (trauma, pneumonia, congestive heart failure), we assessed physician preferences for use and types of fluid and vasopressor medication in pre-EETI resuscitation of critically ill patients.\n \n \n \nResults:\n In total, 1,758 physicians were surveyed (response rate 50.2%, 882/1758). Overall, physicians would perform pre-EETI resuscitation using either fluids or vasopressors in 54% (1,193/2,203) of cases. Most physicians would “always/often” administer intravenous fluid pre-EETI in the three clinical scenarios (81%, 1,484/1,830). Crystalloids were the most common fluid physicians would “always/often” administer in congestive heart failure (EM 43%; CCM 44%), pneumonia (EM 97%; CCM 95%) and trauma (EM 96%; CCM 96%). Pre-EETI resuscitation using vasopressors was uncommon (4.9%). Many suffer from substance use and homelessness. If they use the emergency department (ED) as their primary source of care, potentially preventable frequent ED revisits and hospital readmissions can worsen an already crowded healthcare system. However, the magnitude to which homelessness affects health service utilization among patients with mental health conditions remains unclear in the medical community. This study\n \nassessed the impact of homelessness on 30-day ED revisits and hospital readmissions among patients presenting with mental health conditions in an urban, safety-net hospital.\n \nMethods:\n We conducted a secondary analysis of administrative data on all adult ED visits in 2012 in an urban safety-net hospital. Patient demographics, mental health status, homelessness, insurance coverage, level of acuity, and ED disposition per ED visit were analyzed using multilevel modeling to control for multiple visits nested within patients. We performed multivariate logistic regressions to evaluate if homelessness moderated the likelihood of mental health patients’ 30-day ED revisits and hospital readmissions.\n \nResults:\n Study included 139,414 adult ED visits from 92,307 unique patients (43.5±15.1 years, 51.3% male, 68.2% Hispanic/Latino). Nearly 8% of patients presented with mental health conditions, while 4.6% were homeless at any time during the study period. Among patients with mental health conditions, being homeless contributed to an additional 28.0% increase in likelihood (4.28 to 5.48 odds) of 30-day ED revisits and 38.2% increase in likelihood (2.04 to 2.82 odds) of hospital readmission, compared to non-homeless, non-mental health (NHNM) patients as the base category. Adjusted predicted probabilities showed that homeless patients presenting with mental health conditions have a 31.1% chance of returning to the ED within 30-day post discharge and a 3.7% chance of hospital readmission, compared to non-homeless patients presenting with mental health conditions (25.2%, 2.6%) and NHNM (7.7%, 1.5%).\n \nConclusion:\n Homeless patients presenting with mental health conditions were more likely to return to the ED within 30 days and to be readmitted to the hospital. Our objective was to assess how ONSD measurement varies during T1D-related illness, in order to determine the potential of this tool for discrimination of subclinical DKA-CE.\n \n \n \nMethods: \nWe prospectively enrolled patients aged 7–18 years into three study arms: 1) well-controlled type 1 diabetes; 2) type 1 diabetes with hyperglycemia; 3) DKA. Exclusion criteria included >10 mL/kg of intravenous fluid or insulin prior to transfer, or conditions predisposing to increased intracranial or intraocular pressure. ONSD measurements were obtained within 4h of arrival. One-way ANOVA and multivariable linear regression were used to assess ONSD between groups and association with known DKA-CE risk factors, respectively. Reliability measures were assessed and target enrollment was 36 patients per arm based on sample size calculations.\n \n \n \nResults: \nWe enrolled 108 patients.\n \nNo patients had clinically overt DKA-CE. The between group difference in mean ONSD (mm ± SD) among patients with well-controlled type 1 diabetes (5.2 ± 0.85), T1D with hyperglycemia (5.0 ± 0.91), and DKA (5.2 ± 0.92) was not significant (p=0.79). Mean ONSD was not independently associated with presenting laboratory parameters, known DKA-CE risk factors, or time to ultrasound. Demographic and treatment variables were collected using a chart reviewof 113 ED patient visits with a billing diagnosis of “nausea and vomiting in pregnancy” or “hyperemesisgravidarum.” Logistic regression analysis was used with a primary outcome of return visit to the ED for thesame diagnoses.\nResults:\n There was wide treatment variability of nausea and vomiting in pregnancy patients in the ED.Of the 113 patient visits, 38 (33.6%) had a return ED visit for NVP. High gravidity (OR 1.31, 95% CI [1.06-1.61]), high parity (OR 1.50 95% CI [1.12-2.00]), and early gestational age (OR 0.74 95% CI [0.60-0.90])were associated with an increase in return ED visits in univariate logistic regression models, while onlyearly gestational age (OR 0.74 95% CI [0.59-0.91]) was associated with increased return ED visits in amultiple regression model. Patients present with acute psychosis, memory impairment, dyskinesias, seizures, and/or speech disorders. The clinical course is often complicated by respiratory failure, requiring intubation. Approximately half of patients are found to have an associated ovarian tumor, which expresses NMDAR (Tuzun). Recognition of anti-NMDAR encephalitis by emergency physicians is essential in order to initiate early treatment and avoid psychiatric misdiagnosis. The disease is highly treatable with tumor removal and immunosuppression, and most patients demonstrate a full recovery. Paramedics received education on systemic inflammatory response syndrome (SIRS) criteria, were trained in the use of TATs and hand-held lactate meters, and enrolled patients who had a recent history of infection, met ≥ 2 SIRS criteria, and were being transported to a participating hospital. Blood lactate was measured by paramedics in the prehospital setting and again in the emergency department (ED) via usual care. Paramedics entered data using an online database accessible at the point of care.\n \n \n \nResults:\n Prehospital lactate values obtained by paramedics ranged from 0.8 to 9.8 mmol/L, and an elevated lactate (i.e. ≥ 4.0) was documented in 13 of 112 enrolled patients (12%). The unadjusted correlation of prehospital and ED lactate values was 0.57 (p< 0.001). The median interval between paramedic assessment of blood lactate and the electronic posting of the ED-measured lactate value in the hospital record was 111 minutes. Overall, 91 patients (81%) were hospitalized after ED evaluation, 27 (24%) were ultimately diagnosed with sepsis, and 3 (3%) died during hospitalization. Subjects with elevated prehospital lactate were somewhat more likely to have been admitted to the intensive care unit (23% vs 15%) and to have been diagnosed with sepsis (38% vs 22%) than those with normal lactate levels, but these differences were not statistically significant.\n \n \n \nConclusion:\n In this pilot, EMS use of a combination of objective SIRS criteria, subjective assessment of infection, and blood lactate measurements did not achieve a level of diagnostic accuracy for sepsis that would warrant hospital prenotification and committed resources at a receiving hospital based on EMS assessment alone. Nevertheless, this work provides an early model for increasing EMS awareness and the implementation of novel devices that may enhance the prehospital assessment for sepsis. We simulated a 35-year period, spanning from January 1, 1980 through December 31, 2014. This simulation was used to develop salinity distribution maps to facilitate visualization of fish distribution and abundance data. We compared predicted salinity from the coarse-grid UnTRIM Bay–Delta model to continuous salinity monitoring observations as well to the measured surface salinity from San Pablo Bay through the Delta at a total of 5,542 times and locations where surface salinity was observed as part of several long-term fish monitoring programs: the Fall Midwater Trawl, Summer Townet Survey, and San Francisco Bay Study. The coarse-grid UnTRIM Bay–Delta model was shown to accurately predict hydrodynamics and the spatial distribution of salinity over both a 3-year detailed validation period and over the full 35-year analysis period. The predicted salinity was used to calculate the daily position of X2 and the daily-averaged area of the Low Salinity Zone (LSZ) for each day during the 35-year simulation. Our analysis highlights the influence of multi-year climate patterns, shorter-duration weather patterns, and Delta outflow on salinity distribution. We used the predicted salinity to develop maps of salinity distribution over seven periods for six fish species, and combined the salinity maps with historic fish sampling data to allow for visualization of fish abundance and distribution for 33 years between 1980 and 2012. In the decades before 2006, tools available to quantify growth, dispersal, and survival of juvenile salmon in this complex channel network were limited.Fortunately, thanks to technological advances such as acoustic telemetry and chemical and structural otolith analysis, much has been learned over the past decade about the role of the Delta in the life cycle of juvenile salmon. Here, we review new science between 2006and 2016 that sheds light on how different life stages and runs of juvenile salmon grow, move, and survive in the complex channel network of the Delta. One of the most important advances during the past decade has been the widespread adoption of acoustic telemetry techniques. Use of telemetry has shed light on how survival varies among alternative migration routes and the proportion of fish that use each migration route. Chemical and structural analysis of otoliths has provided insights about when juveniles left their natal river and provided evidence of extended rearing in the brackish or saltwater regions of the Delta. New advancements in genetics now allow individuals captured by trawls to be assigned to specific runs. Detailed information about movement and survival in the Delta has spurred development of agent-based models of juvenile salmon that are coupled to hydrodynamic models. Although much has been learned, knowledge gaps remain about how very small juvenile salmon (fry and parr) use theDelta. Managed aquifer recharge (MAR) projects can play an important role in ensuring California manages its groundwater sustainably. Although the benefits and economic costs of surface water storage have been researched extensively, the benefits and economic costs of MAR have been little researched. Historical groundwater data are sparse or proprietary within the state, often impairing groundwater analyses. General obligation bonds from ballot propositions offer a strategic means of mining information about MAR projects, because the information is available publicly. We used bond-funding applications to identify anticipated MAR project benefits and proposed economic costs. We then compared these costs with actual project costs collected from a survey, and identified factors that promote or limit MAR. Our analysis indicates that the median proposed economic cost for MAR projects in California is $410 per acre-foot per year ($0.33 per cubic meter per year). Increasing Water Supply, Conjunctive Use, and Flood Protection are the most common benefits reported. Additionally, the survey indicates that (1) there are many reported reasons for differences between proposed and actual costs ($US 2015) and (2) there is one primary reason for differences between proposed recharge volumes and actual recharge volumes (AFY): availability of source water for recharge. Although there are differences between proposed and actual costs per recharge volume ($US 2015/AFY), the ranges for proposed costs per recharge volume and actual costs per recharge volume for the projects surveyed generally agree. The Delta Smelt is endemic to the upper San Francisco Estuary. Much of its historic habitat is no longer available and remaining habitat is increasingly unable to sustain the population. As a listed species living in the central node of California’s water supply system, Delta Smelt has been the focus of a large research effort to understand causes of decline and identify ways to recover the species. Since 2008, a remarkable record of innovative research on Delta Smelt has been achieved, which is summarized here. Unfortunately, research has not prevented the smelt’s continued decline, which is the result of multiple, interacting factors. A major driver of decline is change to the Delta ecosystem from water exports, resulting in reduced outflows and high levels of entrainment in the large pumps of the South Delta. Invasions of alien species, encouraged by environmental change, have also played a contributing role in the decline. Severe drought effects have pushed Delta Smelt to record low levels in 2014–2015. The rapid decline of the species and failure of recovery efforts demonstrate an inability to manage the Delta for the “co-equal goals” of maintaining a healthy ecosystem and providing a reliable water supply for Californians. This paper analyzes the extent to which contributions of the wettest days to overall precipitation dictate the state’s precipitation seasonality and frequent multiyear periods of drought (as precipitation deficit) and plenty is analyzed, historically and in projections of future climates. The wettest 5% of wet days in California contribute about a third of precipitation but about two-thirds of the variance of water-year precipitation. Year-to-year fluctuations in precipitation strongly reflect year-to-year fluctuations of contributions from the largest storms, with the large-storm contributions explaining about twice as much precipitation fluctuation as do contributions from all remaining storms combined. This extreme dominance of large storms is largely unique to California within the United States. In climate-change projections, eight of ten climate models considered here yield increases in precipitation from the largest storms, and when the increases are large, total precipitation follows suit. All of the models project declines in contributions from the smaller storms and models projecting total-precipitation declines reflect this decline. Projected changes in variance of water-year precipitation reflect changes in variance of large-storm contributions. I reviewed relevant predator–prey theory, and described extant data on predator–prey relationships of Delta fishes. I ranked predator consumption rates as occasional, moderate, and common, based on frequency-of-occurrence data, and evaluated the frequency, and hypothesized the effects of predation on native and invasive species. I identified 32 different predator categories and 41 different prey categories. Most predators were occasional consumers of individual prey species, although I also observed moderate and common consumption of some prey types. My analysis yielded few generalizations regarding predator–prey interactions for Delta fishes; most predators consumed a variety of both native and invasive fishes. The only evidence for predator specialization on either native or invasive fishes occurred in Prickly Sculpin which, when it consumed fishes, ate mostly native species. Both Striped and Largemouth Bass exhibited wide dietary breadth, preying upon 32 and 28 categories of fish prey respectively. Sacramento Pikeminnow, a native predator, also displayed wide dietary breadth of piscine prey, with 14 different prey categories consumed. Data for reptilian, avian, and mammalian predators were sparse; however, these predators may be significant fish predators in altered habitats or when hatchery salmonids are released. The database for predators and their fish prey was not strong, and I recommend long-term dietary studies combined with prey availability and behavioral and experimental studies to establish predator preferences and anti-predator behaviors, rather than just consumption. The behavioral effects of contaminants on prey species also warrant further examination. Consequently, management and restoration that focus on individual places may fail to recognize and incorporate interactions across entire landscapes. The science of landscape ecology, which emphasizes the interplay of landscape structure, function, and change at multiple scales, offers a perspective that can integrate the spatial relationships of ecological processes and the functional interconnections of land and water in the Delta. Although the Delta is one of the most studied estuaries in the world, applications of landscape science have been limited. We describe why it is important to incorporate landscape science into management and restoration, emphasizing how Delta landscapes have changed over the past centuries. The land–water linkages of the past have been broken, waterways have been over-connected, and hard boundaries have replaced the gradual and dynamic transitions among landscape patches. The contemporary landscape also has new, novel assemblages of species and stressors that were not there in the past. This historical perspective indicates how knowledge of past landscape functions can contribute to the restoration and management of contemporary landscapes. We illustrate these points with case studies of inundation dynamics and riparian woodlands, and use a third example to describe a landscape approach to restoration. We propose that science that encompasses the multiple, interacting components of functional landscapes in the Delta will foster resilient and enduring restoration and management outcomes that benefit both people and wildlife. The frequency and occurrence of seismic activity in the central and eastern United States has increased dramatically since the shale boom began in 2009. This uptick in seismic events has led many to suspect that the connection between unconventional production and induced seismicity is far from coincidence. Plenty of myths and inaccurate reporting surround this issue, but the consensus from the scientific community is that the injection of wastewater fluids is the most likely culprit for the increasing rates of seismicity. This concern has prompted a varied response from concerned citizens, regulators, and the industry. This article discusses how the oil and gas industry induces earthquakes from wastewater disposal activities, outlines the existing regulatory framework by comparing the response of state officials in Oklahoma and Colorado, and offers several non-regulatory strategies that companies should implement to prevent these damaging earthquakes. In February 2015, the United States Department of the Interior released a proposed rule employing more stringent standards and requirements for offshore exploratory drilling operations in order to ensure effective and safe exploration in the Arctic Outer Continental Shelf. The proposed rule has received mixed responses, with environmental groups praising the government for regulating such activities while simultaneously requesting that the mandates become even more stringent. The regulated entities in the oil and gas industry oppose the rule because they argue it is redundant and unnecessary, while also imposing extravagant costs for relatively minimal improvements to the safety of these exploratory drilling operations. However, as with any regulation, controversy among different interest groups is to be expected. To determine whether the proposed rule is the appropriate regulatory approach, this article employs a familiar tool to determine the favorability of the proposed regulation: economic analysis. Although some claim that economic analysis is an improper tool for quantifying certain benefits such as environmental protection, it seems clear that the government must find some way to reconcile these conflicting interests and ensure the sustainable development of Arctic resources moving forward. Most of the sources are the more or less fragmentary lists of wages/payments that reflect various conditions, such as status of the recipients, period to which the payment corresponds etc, that are not always known to us. Other documents provide us with categories of allowances ascribed to the workmen and officials who participated on the same project. A few traces of a systematic approach can be recognized in the evidence, for instance value-units and day’s work units, but many details remain unclear. Bread, beer and grain represented the basic components of the rations in all periods. Bread and beer was often allocated daily while the grain was at some periods used as a monthly payment. On the other hand meat was considered an extra ration while linen and other valuable products could be distributed in longer periods, for instance once a year. International treatment guidelines for sepsis advocate that treatment be initiated at the earliest possible opportunity. It is not yet clear if very early intervention by ambulance clinicians prior to arrival at hospital leads to improved clinical outcomes among sepsis patients.\nMethods: \nWe systematically searched the electronic databases MEDLINE, EMBASE, CINAHL, the Cochrane Library and PubMed up to June 2015. In addition, subject experts were contacted. We adopted the GRADE (grading recommendations assessment, development and evaluation) methodology to conduct the review and follow PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) recommendations to report findings.\nResults:\n Nine studies met the eligibility criteria – one study was a randomized controlled trial while the remaining studies were observational in nature. There was considerable variation in the methodological approaches adopted and outcome measures reported across the studies. Because of these differences, the studies did not answer a unique research question and meta-analysis was not appropriate. A narrative approach to data synthesis was adopted.\nConclusion: \nThere is little robust evidence addressing the impact of prehospital interventions on outcomes in sepsis. That which is available is of low quality and indicates that prehospital interventions have limited impact on outcomes in sepsis beyond improving process outcomes and expediting the patient’s passage through the emergency care pathway. Emergency physicians increasingly employ ultrasound in the evaluation of these complaints. As a result, it is likely that rare, though important, diagnoses will be encountered. We report a case of fetal anencephaly diagnosed by bedside emergency ultrasound in a patient presenting with first trimester vaginal bleeding.\n \nCase Report: \nA 33 year-old patient at 10 weeks gestation presented with vaginal bleeding. After initial history and physical examination, a bedside ultrasound was performed. The emergency physician noted the abnormal appearance of the fetal cranium and anencephaly was suspected. This finding was confirmed by a consultative high-resolution fetal ultrasound. Making the diagnosis at the point of care allowed earlier detection, and more comprehensive maternal counseling about pregnancy options. This particular patient underwent elective abortion which was able to be performed at an earlier gestation, decreasing maternal risk.
