In this article, I use CA to analyze student initiatives (Waring, 2011) within an IRF sequence in one excerpt from a Chinese as a foreign language class. The excerpt features a teacher using an IRF sequence to engage her students in a sentence-based translation exercise. I demonstrate how a student initiates a sequence following the teacher’s feedback move to make a negative assessment of the pragmatic soundness of the sentences, thus casting doubt on the teacher's epistemic authority. This initiating action and the subsequent interaction it generates bring contingency into the IRF sequence and create potential learning opportunities. As the teacher contends with the contingency, issues related to epistemic asymmetry and L1 and L2 identities are brought to the surface. Additionally, the potential opportunities to discuss the different pragmatic forces between \nyao\n in Chinese and \nto want\n in English when used in making requests are missed at multiple sequential junctures. Based on the analysis, I discuss teacher-student epistemic social relations within the IRF sequence and a methodological issue concerning the analysis of missed learning opportunities. Over 200 responses were received from 28 different countries representing scientists from many different backgrounds and institutions. Top priorities for research interests were clearly divided by the setting in which scientists conducted their research with animals. Experts from the fields identified as the most important future research topics on comparative psychology were invited to review their respective fields for publication trends over the last 20 years. The results of their significant efforts and the importance of continuing research in comparative psychology are presented and addressed in this special issue. While most people would assume that mainstreaming is beneficial for deaf and hard-of hearing students, little is known about their academic success. Current information claims that when D/HH students are accepted in mainstream schools they have better academic success, while other research claims that acceptance has no effect on the academic success of deaf and hard-of-hearing students. This article will identify the problems in the available studies and show why there is yet no clear answer on whether or not acceptance of deaf and hard-of-hearing students by their hearing peers and teachers has any effects on their academic success. Mark Twain, noted as the father of American literature, wrote the book \nRoughing It\n, a semi-autobiographical work that tells of his experiences in the western part of the United States, from the Nevada to California territories, during the years 1861 to 1867. John Muir, an advocate for wilderness preservation and founder of the Sierra Club, published his own diary entries in the book, \nMy First Summer in the Sierra\n, recounting the sensations nature brought to him and the specific findings he discovered as a naturalist during 1869. Each of these renowned pieces of literature not only gives a description of the landscape and the authors’ time living in it, but also a reflection on the discoveries and emotions that accompany it. Through the works of Twain and Muir that describe their adventures pioneering the Sierra Nevada mountain range during the mid 19th century, the compelling descriptions toward an unfamiliar place promoted the concepts of the adventurous spirit of pioneering, the development of a relationship with nature, and the American character. Upon leaving these contexts, the resting EEG patterns found in post-institutionalized (PI) children resemble the EEG profile of children with behavior problems, such as inattention, hyperactivity, and impulsivity. Specifically, this atypical pattern consists of increased theta power relative to other power spectra and decreased alpha power. This study examined if this pattern persists years after adoption and/or whether they were found only in PI youth who developed ADHD. PI and nonadopted (NA) children, between 11-15 year old, comprised the two groups in the study. Approximately half of the participants in each group were diagnosed with ADHD. Parent-reported executive function and attention problems differed by ADHD, but not by adoption classification (PI/NA). The PI youth and NA youth with ADHD exhibited the atypical EEG pattern related to early deprivation and ADHD-related deficits. A review of current research in the medical field", "subtitle": null, "abstract": "The ADK hypothesis and the disruption of Ca2+ homeostasis in the brain are leading causes of epileptogenesis. These are the main underlying factors that lead to post-stroke epilepsy. In addition to this, epilepsy is known to cause a significant increase in risk for depression, which has been shown to be coupled with attenuation of the hippocampal region. This knowledge would lead one to think that epilepsy should be treatable, but current treatments do not address all of the issues at hand. The newest treatment, Levetiracetam, has been shown to reduce expression of synaptic vesicle protein 2A in order to control Ca2+ regulation but its side effects in terms of depression are controversial. Following is a culmination of new progress in treating epilepsy. I propose that if ostracism can cause cognitive and behavioral patterns that resemble those of ADHD, then it is possible that prolonged ostracism (especially early in life) can exacerbate risk for developing ADHD. These characteristics attracted a large fandom. The fandom’s demographics are especially interesting. A very significant portion of the fandom is young adult males (known as Bronies) usually around college age, not the target audience at all and yet they are some of the most devoted fans. This paper analyses lyrics from some of the music created by these fans. These songs show that these fans are resisting and redefining what it means to be male within their subculture. The songs mentioned are from popular musicians within the fandom and their popularity is indicative of the ideals valued by the community. Their songs are about being emotionally vulnerable and being unafraid to ask for help when one needs it. The rather feminine nature of the show and the unorthodox fandom is resisting the traditional views on males constructed by hegemonic ideals, as laid out by Dr. Mike Donaldson, namely the idea that they must be aggressively independent and unfeeling. According to Dr. Donaldson, a professor of media & communication at the University of Wollongong in Australia who specializes in the study of masculinity, hegemonic masculinity deals with the ideas that a man must be tough and further the subordination of women. Frick refused to entertain any sort of negotiations with the union workers of the mill and in response the workers decided to strike. Frick hired the Pinkerton detectives to break up the strike and regain control of the mill. The arriving Pinkertons not only faced a mob of steel workers that night but also the weapons the workers managed to scrap together. Expanded recoveries are used to derive important estimates such as the total number and proportion of hatchery-origin fish present. The standard estimation scheme assumes accurate visual classification of adc status, which can be problematic for decomposing carcasses. Failure to account for this potential misclassification can lead to significant estimation bias. We reviewed sample expansion factors used for the California Central Valley Chinook salmon 2010 carcass surveys in this context. For upper Sacramento River fall-run and late fall-run carcass surveys, the estimated proportions of adc,cwt fish for fresh and non-fresh carcasses differed substantially, likely from the under-recognition of adc fish in non-fresh carcasses. The resulting estimated proportions of hatchery-origin fish in the upper Sacramento River fall-run and late fall-run carcass surveys were 2.33 to 2.89 times higher if only fresh carcasses are considered. Similar biases can be avoided by consideration of only fresh carcasses for which determination of adc status is relatively straightforward; however, restricting the analysis entirely to fresh carcasses may limit precision because of reduced sample size, and is only possible if protocols for sampling and recording data ensure that the sample data and results for fresh carcasses can be extracted. While 2-D and 3-D hydrodynamic and water quality models are by definition better suited to modeling a complex network of tidally influenced flows under future conditions, a 1-D model is more computationally efficient in narrowing the large variety of multiple-year simulations required into a more manageable task. Still, a 1-D model of sea level rise in an estuary must account for the three-dimensional effects where increased depths will affect density driven (baroclinic) circulation and tidal dispersion of salt. In this paper, we use a simplified Delta network model with a tidally averaged computational approach to quickly perform multi-year simulations for sea level rise. The 1-D model uses tidal dispersion coefficients developed from 3-D hydrodynamic models. The resulting model is capable of performing very fast simulations over a wide range of conditions, providing guidance on what should be explored in depth with more detailed, but slower models.\n \nComparisons of unimpaired Delta inflow with the historical case show that the south Delta and San Joaquin River would be much fresher without exports, while the Sacramento River would be fresher in spring and more saline in the fall. Sea level rise will increase salinity throughout the Delta over time. With peripheral conveyance of export, water salinity will intrude upstream in the Sacramento River, be slightly lower up the San Joaquin River and increase in the south Delta. Our particular focus was the covariation of flow with the spatial extent of the low-salinity zone (LSZ: salinity = 0.5 to 6), and with the extent of habitat for common species of -nekton as defined by their salinity ranges. The UnTRIM model has an unstructured grid which allowed us to refine earlier estimates of the availability of suitable salinity ranges, particularly for species resident in low salinity. The response of the salinity field to flow was influenced by the bathymetry of the estuary. Area and volume of the LSZ were bimodal with X2, the distance up the axis of the estuary to a near-bottom salinity of 2, roughly the middle of the LSZ. The smallest area and volume occurred when the LSZ was in the Delta or Carquinez Strait, moderate values when it was in Suisun Bay, and the highest values when it was in broad, shallow San Pablo Bay. Resource selection functions for the distributions of common nekton species in salinity space were up-dated from previous values and used to calculate salinity-based habitat indices using the UnTRIM results. These indices generally increased with decreasing X2 (increasing flow), but the slopes of these relationships were mostly inconsistent with corresponding relationships of abundance to flow. The purpose of this study was to characterize medical students’ exposure to defensive medicine during medical school rotations. Methods: We performed a cross- sectional survey study of medical students at the beginning of their third year. We gave students Likert scale questionnaires, and their responses were tabulated as a percent with 95% confidence interval (CI).Results: Of the 124 eligible third-year students,102 (82%) responded. Most stated they rarely worried about being sued (85.3% [95% CI=77.1% to 90.9%]). A majority felt that faculty were concerned about malpractice (55.9% [95% CI=46.2% to 65.1%]), and a smaller percentage stated that faculty taught defensive medicine (32.4% [95% CI=24.1% to 41.9%]). Many students believed their satisfaction would be decreased by MC and lawsuits (51.0% [95% CI=41.4% to 60.5%]). Some believed their choice of medical specialty would be influenced by MC (21.6% [95% CI=14.7% to 30.5%]), and a modest number felt their enjoyment of learning medicine was lessened by MC (23.5% [95% CI=16.4% to 32.6%]). We describe a residency research program, taking advantage of environmental change aligning resident and faculty goals, to become a contributor to departmental cultural change and research development.\nMethods: A research program, Scholar Quest (SQ), was developed as a part of an Information Mastery program. The goal of SQ is for residents to gain understanding of scholarly activity through a mentor-directed experience in original research. This curriculum is facilitated by providing residents protected time for didactics, seed grants and statistical/staff support. We evaluated total scholarly activity and resident/faculty involvement before and after implementation (PRE-SQ; 2003-2005 and POST-SQ; 2007-2009).\nResults: Scholarly activity was greater POST-SQ versus PRE-SQ (123 versus 27) (p<0.05) with an incidence rate ratio (IRR)=2.35. Resident and faculty involvement in scholarly activity also increased PRE-SQ to POST-SQ (22 to 98 residents; 10 to 39 faculty, p<0.05) with an IRR=2.87 and 2.69, respectively.\nConclusion: Implementation of a program using department environmental change promoting a resident longitudinal research curriculum yielded increased resident and faculty scholarly involvement, as well as an increase in total scholarly activity. Accuracy of Patient Estimates", "subtitle": null, "abstract": "Introduction: Managing a patient’s expectations in the emergency department (ED) environment is challenging. Previous studies have identified several factors associated with ED patient satisfaction. Lengthy wait times have shown to be associated with dissatisfaction with ED care. Understanding that patients are inaccurate at their estimation of wait time, which could lead to lower satisfaction, provides administrators possible points of intervention to help improve accuracy of estimation and possibly satisfaction with the ED. This study was undertaken to examine the accuracy of patient estimates of time periods in an ED and identify factors associated with accuracy.Method: In this prospective convenience sample survey at UTMC ED, we collected data between March and July 2012. Outcome measures included duration of each phase of ED care and patient estimates of these time periods.Results: Among 309 participants, the majority underestimated the total length of stay (LOS) in the ED (median difference -7 minutes (IQR -29-12)). There was significant variability in ED LOS (median 155 minutes (IQR 75-240)). No significant associations were identified between accuracy of time estimates and gender, age, race, or insurance status. Participants with longer ED LOS demonstrated lower patient satisfaction scores (p<0.001).Conclusion: Patients demonstrated inaccurate time estimates of ED treatment times, including total LOS. Patients with longer ED LOS had lower patient satisfaction scores. However, it is costly, time-consuming and exposes patients to significant doses of ionizing radiation. Hydronephrosis on bedside ultrasound is a sign of a ureteral stone, and has a reported sensitivity of 72-83% for identification of unilateral hydronephrosis when compared to CT. The purpose of this study was to evaluate trends in sensitivity related to stone size and number. Methods: This was a structured, explicit, retrospective chart review. Two blinded investigators used reviewed charts of all adult patients over a 6-month period with a final diagnosis of renal colic. Of these charts, those with CT evidence of renal calculus by attending radiologist read were examined for results of bedside ultrasound performed by an emergency physician. We included only those patient encounters with both CT-proven renal calculi and documented bedside ultrasound results. Results: 125 patients met inclusion criteria. The overall sensitivity of ultrasound for detection of hydronephrosis was 78.4% [95% confidence interval (CI)=70.2-85.3%]. The overall sensitivity of a positive ultrasound finding of either hydronephrosis or visualized stones was 82.4% [95%CI: 75.6%, 89.2%]. Based on a prior assumption that ultrasound would detect hydronephrosis more often in patients with larger stones, we found a statistically significant (p = 0.016) difference in detecting hydronephrosis in patients with a stone ≥6mm (sensitivity=90% [95% CI=82-98%]) compared to a stone <6mm (sensitivity=75% [95% CI=65-86%]). For those with 3 or more stones, sensitivity was 100% [95% CI=63-100%]. Several studies show correlation of sonographic optic nerve sheath diameter (ONSD) to eICP, while others show high inter-observer variability or marginal performance with less experienced sonographers. The objective of our study is to assess the ability of bedside ultrasound measurement of ONSD to identify the presence of eICP when performed by a single experienced sonographer. We hypothesize that ONSD measurement is sensitive and specific for detecting eICP and can be correlated with values obtained by external ventricular device (EVD).\nMethods: This was a prospective blinded observational study conducted in a neurocritical care unit of a level 1 trauma center. ONSD measurement was performed on a convenience sample of 27 adult patients who required placement of an invasive intracranial monitor as part of their clinical care. One certified sonographer/physician performed all ultrasounds within 24 hours of placement of EVD. The sonographer was blinded to the ICP recorded by invasive monitor at the time of the scan. A mean ONSD value of ≥5.2 mm was taken as positive.\nResults: The sonographer performed 27 ocular ultrasounds on individual patients. Six (22%) of these patients had eICP (EVD measurement of >20 mmHg). Spearman rank correlation coefficient of ONSD and ICP was 0.408 (p=0.03), demonstrating a moderate positive correlation. A ROC curve was created to determine the optimal cut off value to distinguish an eICP greater than 20 mmHg. The area under the receiver operator characteristic curve was 0.8712 (95% confidence interval [CI]=0.67 to 0.96). Walk-in STEMI patients have longer treatment times compared to emergency medical service (EMS) transported patients. While effective triage of large numbers of critically ill patients in the emergency department is often cited as the reason for treatment delays, additional factors have not been explored. The purpose of this study was to evaluate baseline demographic and clinical differences between walk-in and EMS-transported STEMI patients and identify factors associated with prolonged door to balloon (D2B) time in walk-in STEMI patients.\nMethods: We performed a retrospective review of 136 STEMI patients presenting to an urban academic teaching center from January 2009 through December, 2010. Baseline demographics, mode of hospital entry (walk-in versus EMS transport), treatment times, angiographic findings, procedures performed and in-hospital clinical events were collected. We compared walk-in and EMS-transported STEMI patients and identified independent factors of prolonged D2B time for walk-in patients using stepwise logistic regression analysis.\nResults: Walk-in patients (n=51) were more likely to be Latino and presented with a higher heart rate, higher systolic blood pressure, prior history of diabetes mellitus and were more likely to have an elevated initial troponin value, compared to EMS-transported patients. EMS-transported patients (n=64) were more likely to be white and had a higher prevalence of left main coronary artery disease, compared to walk-in patients. Door to electrocardiogram (ECG), ECG to catheterization laboratory (CL) activation and D2B times were significantly longer for walk-in patients. Walk-in patients were more likely to have D2B time > 90 minutes, compared to EMS- transported patients; odds ratio 3.53 (95% CI 1.03, 12.07), p = 0.04. Stepwise logistic regression identified hospital entry mode as the only independent predictor for prolonged D2B time.\nConclusion: Baseline differences exist between walk-in and EMS-transported STEMI patients undergoing primary PCI. Hospital entry mode was the most important predictor for prolonged treatment times for primary PCI, independent of age, Latino ethnicity, heart rate, systolic blood pressure and initial troponin value. Prolonged door to ECG and ECG to CL activation times are modifiable factors associated with prolonged treatment times in walk-in STEMI patients. Patients with Non-specific Abdominal Pain", "subtitle": null, "abstract": "Introduction: Patients with abdominal pain often return multiple times despite no definitive diagnosis. Our objective was to determine if repeat emergency department (ED) use among patients with non-specific abdominal pain might be associated with a diagnosis of moderate to severe depressive disorder.\nMethods: We screened 987 ED patients for major depression during weekday daytime hours from June 2011 through November 2011 using a validated depression screening tool, the PHQ-9. Each subject was classified as either no depression, mild depression or moderate/ severe depression based on the screening tool. Within this group, we identified 83 patients with non-specific abdominal pain by either primary or secondary diagnosis. Comparing depressed patients versus non-depressed patients, we analyzed demographic characteristics and number of prior ED visits in the past year.\nResults: In patients with non-specific abdominal pain, 61.9% of patients with moderate or severe depression (PHQ9≥10) had at least one visit to our ED for the same complaint within a 365-day period, as compared to 29.2% of patients with no depression (PHQ9<5), (p=0.013).\nConclusion: Repeat ED use among patients with non-specific abdominal pain is associated with moderate to severe depressive disorder. Patients with multiple visits for abdominal pain may benefit from targeted ED screening for depression.
