EJ scientists continue to shape and reshape the meaning of EJ. Meanwhile, activists also use rhetoric boundaries to discredit the legitimacy of EJ’s opponents. We suggest that the EJ movement is still dependent on scientists to provide a scientific way to foster equal distribution of environmental risks. However, to achieve a just distribution, science along is not enough. Using existing computed tomography (CT) decision rules for selecting patients to be referred for CT, such as the New Orleans Criteria (NOC), approximately 70% of those scanned are found to have a negative CT. This study investigates the use of quantified brain electrical activity to assess its possible role in the initial screening of ED mTBI patients as compared to NOC.\nMethods: We studied 119 patients who reported to the ED with mTBI and received a CT. Using a hand-held electroencephalogram (EEG) acquisition device, we collected data from frontal leads to determine the likelihood of a positive CT. The brain electrical activity was processed off-line to generate an index (TBI-Index, biomarker). This index was previously derived using an independent population, and the value found to be sensitive for significant brain dysfunction in TBI patients. We compared this performance of the TBI-Index to the NOC for accuracy in prediction of positive CT findings.\nResults: Both the brain electrical activity TBI-Index and the NOC had sensitivities, at 94.7% and 92.1% respectively. The specificity of the TBI-Index was more than twice that of NOC, 49.4% and 23.5% respectively. The positive predictive value, negative predictive value and the positive likelihood ratio were better with the TBI-Index. When either the TBI-Index or the NOC are positive (combining both indices) the sensitivity to detect a positive CT increases to 97%.\nConclusion: The hand-held EEG device with a limited frontal montage is applicable to the ED environment and its performance was superior to that obtained using the New Orleans criteria. This study suggests a possible role for an index of brain function based on EEG to aid in the acute assessment of mTBI patients. Such testing may represent an unnecessary expense that prolongs emergency department stays without significantly improving care. This study determines the prevalence of such testing and how often it leads to changes in care.\nMethods: We prospectively tracked laboratory testing among psychiatric patients presenting to the emergency departments of two academic tertiary care facilities. For each visit we determined whether laboratory or radiographic studies were ordered, and whether the examination was conducted at the request of the emergency physician as part of a medical screening examination or requested by the psychiatry service. We then determined if this testing changed patient disposition.\nResults: Our study enrolled 598 patients. Of these, emergency physicians ordered testing as a part of medical screening on 155 patients (25.9%). We found the psychiatry service ordered laboratory or radiographic studies for 191 of 434 patients (44.0%) who emergency physicians determined did not require ancillary testing for medical clearance. Of these 191 patients, only one (0.5%; 95% Confidence Interval: 0.01% - 2.9%) had an abnormal result that led to a change in disposition. Total Medicare reimbursement rates for the additional ancillary testing in this study was $37,682.\nConclusion: Ancillary testing beyond what is required for medical clearance of psychiatric emergency patients rarely alters care. Policies that require panels of testing prior to psychiatric admission are costly and appear to be unnecessary. Previous investigations have examined funding and productivity in EM research, but whether EM researchers preferentially concentrate on certain patient-related topics is notknown. We hypothesized that at least part of the scarcity of EM research is from the tendency of EM researchers, like researchers in other fields, to focus on rarer conditions with higher morbidity or mortality instead of on more common conditions with less acuity. This study compared the frequency of specific medical conditions presenting to emergency departments nationwide with the frequency of emergency physician research on those same conditions.\nMethods: This study is a structured retrospective review and comparison of 2 databases during an 11-year span. Principal diagnoses made by emergency physicians as reported by the National HospitalAmbulatory Medical Care Survey were compared to all first-author publications by emergency physicians as reported in PubMed between 1996 and 2006. Statistics included correlations and linear regression with the number of emergency department (ED) visits per diagnosis as the independent variable and the number of articles published as the dependent variable.\nResults: During the study period, there was significant concordance between the frequency of presenting conditions in the emergency department and the frequency of research being performed onthose conditions, with a high correlation of 0.85 (P,0.01). More common ED diagnoses such as injury/poisoning, symptoms/ill-defined conditions, and diseases of the respiratory system accounted for 60.9% of ED principal diagnoses and 50.2% of the total research published in PubMed.\nConclusion: Unlike researchers in other fields, emergency physicians investigate clinical problems in almost the exact proportion as those conditions are encountered in the emergency department. Ultrasound (US) has not previously been used as a means to confirm intrathoracic placement of chest tubes. This study involves a novel application of US to demonstrate chest tubes passing through the pleural line, thus confirming intrathoracic placement.\nMethods: This was an observational proof-of-concept study using a convenience sample of patients with TTs at a tertiary-care university hospital. Bedside US was performed by the primary investigatorusing first the low-frequency (5–1 MHz) followed by the high-frequency (10–5 MHz) transducers, in both 2-dimensional gray-scale and M-modes in a uniform manner. The TTs were identified in transverse and longitudinal views by starting at the skin entry point and scanning to where the TT passed the pleural line, entering the intrathoracic region. All US images were reviewed by US fellowship-trained emergency physicians. CXRs and CTs were used as the standard for confirmation ofTT placement.\nResults: Seventeen patients with a total of 21 TTs were enrolled. TTs were visualized entering the intrathoracic space in 100% of cases. They were subjectively best visualized with the high-frequency (10–5 MHz) linear transducer. Sixteen TTs were evaluated using M-mode. TTs produced a distinct pattern on M-mode.\nConclusion: Bedside US can visualize the TT and its entrance into the thoracic cavity and it can distinguish it from the pleural line by a characteristic M-mode pattern. Alexandria, quite a cosmopolitan city during this time, was no exception, and a home for Jews, Christians and pagans alike. In Alexandria, interactions between these groups ranged from peaceful, scholastic exchange at academies to outbreaks of public violence, most notably the destruction of the Serapeum in 391. Further evidence of the tense interaction between these groups can be found in an examination of the city’s intellectual history: namely, Christians’ fraught relationship with the classical tradition. On one hand, Homer and other classical authors for over a century had formed the educational canon; on the other, these pagan works contained what Christians believed to be questionable morals.\n \nIn this paper I examine the use of the logic tradition in the polemical writings of Cyril of Alexandria. Logic was developed primarily by Aristotle in the Organon and further expanded by his followers and later philosophers. I argue that Cyril does not use logic out of any great admiration of its efficacy, but merely because it is the tool that his opponents use in their own polemical writings: he essentially attempts to beat his enemies with their own weapons. In nude male statuary, the evolution of carefully sculpted and stylized pubic hair to unbridled tufts reveals the shifting definition of masculinity. No longer valuing the ostentatious pubic ornamentation of aristocrats, the newly founded Greek democracy turns to embrace the pubic hair of the everyman. With this change, every citizen can attain bodily austerity just as he can attain influence in his government. In a true reflection of the Classical ideal, his self-containment endows him with masculine power. He suppresses any potential threat to this power, a mindset not limited to merely his rival men. One also can apply this concept of patriarchal dominance to the practice of female genital depilation; the most powerful and therefore most threatening women remove greater quantities of pubic hair, while the more innocuous females need not practice such depilation. This applies to the goddesses, who lack pubic hair completely; the wives, who take pride in their neatly pruned genitalia; the hetaerai who partially depilate to augment eroticism; and the common slaves, who as harmless property do not groom extensively. The man’s pubic dominance remains unattested, however, in vases that include scenes with other males. While these subjects could threaten the patron with a masculine proliferation of pubic hair, they instead juxtapose him with their relative hairlessness. The two most prominent examples of the trope are Helen and Circe. Helen uses a drug to assuage the grief of Telemachos, while Circe tries to use drugs to seduce and capture Odysseus and his men. Both drugs are described as causing men to forget their homes. At the same time, Helen uses rhetoric to rewrite history, and Circe is mysteriously able to narrate Odysseus’s future. A comparison of the two incidents, and of the women’s other deceptive actions, reveals that these memory-altering drugs are part of a more general pattern of divine women having the ability to manipulate reality, along with the constant threat their beauty poses to the familial stability of mortal men. When the analysis is broadened to include other uses of drugs in the poem—those of the Lotus-Eaters and of Odysseus—their significance becomes more complex. They are associated with the danger of forgetting family, but also with godlike powers forbidden to mortals, and, finally, with the sort of deceitful intelligence and dishonorable fighting techniques employed equally by Helen, Circe, and Odysseus. Thus, an analysis of the instances of drugs in the poem reveals unexpected ideas about the protagonist. The case scenarios involved the followingissues: (1) the need for consent for retrospective chart review when research subjects receive their care after the study is conceived; (2) child assent; (3) individual versus population benefit; and (4) exception from informed consent in emergency resuscitation research. The free-text responses were analyzed and abstracted for recurrent themes.\nResults: Twenty-three of the 45 survey recipients completed the survey, for an overall response rate of 51%. Study participant data were then compared with information recorded by Michigan State Police at the scene of the MVC. Agreement between research participant reports and police-reported data were assessed by using percentage agreement and j coefficients for categorical variables and correlation coefficients for continuous variables.\nResults: There were 97 study participants for whom emergency department interviews and Michigan State Police Report information were available. Correlation coefficients were 0.95 for the time of the collision, and 0.58 for extent of damage to the vehicle. Little is known about providers’ ability to estimate patient satisfaction. We aimed to measure providers’ ability to assess patient satisfaction and hypothesized that providers could accurately estimate overall patient satisfaction.\nMethods: We surveyed ED patients regarding satisfaction with their care. Treating providers completed analogous surveys, estimating patients’ responses. Sexual assault victims and non-English-speaking or severely ill patients were excluded. Satisfaction responses were categorized as ‘‘satisfied’’ or ‘‘not satisfied.’’ Patient satisfaction scores were considered the ‘‘gold standard,’’ and providers’ perceptions of the patient satisfaction were considered tests. Measures of diagnosticaccuracy, such as positive predictive value (PPV) and sensitivity, were used to assess how accurately the provider could estimate his or her patient’s satisfaction.\nResults: Here, 242/457 eligible patients (53%) completed the survey; 227 providers (94%) completed a corresponding survey. Subject-reported overall satisfaction was 96.6%, compared with a provider estimated rate of 94.4%. The sensitivity and PPV of the provider’s estimate of the patient’s satisfaction were 95.2 (95% confidence interval [CI] 91.4, 97.7) and 97.5 (95% CI 94.4, 99.2), respectively, for overall patient satisfaction. The PPV was similar for clarity of communication. The PPV was 78.9 for perceived length of ED stay (99% CI 70.8, 85.6) and 82.6 for quality of pain control (95% CI 68.6, 92.2). Accuracy of attending and resident estimates of patient satisfaction did not differ significantly. The objectives of this study were to assess physician knowledge of, barriers to implementation of, and compliance with the Ottawa Knee Rule in academic emergency departments (EDs), and evaluate whether patient characteristics predict guideline noncompliance.\nMethods: A 10 question online survey was distributed to all attending ED physicians working at three affiliated academic EDs to assess knowledge, attitudes and self-reported practice behaviors relatedto the Ottawa Knee Rule. We also performed a retrospective ED record review of patients 13 years of age and older who presented with acute knee trauma to the 3 study EDs during the 2009 calendar year, and we analyzed ED records for 19 variables.\nResults: ED physicians (n = 47) correctly answered 73.2% of questions assessing knowledge of the Ottawa Knee Rule. The most commonly cited barriers to implementation were “patient expectations” and system issues, such as “orthopedics referral requirement.” We retrospectively reviewed 838 records, with 260 eligible for study inclusion. The rate of Ottawa Knee Rule compliance was retrospectively determined to be 63.1%. Ninety-seven percent (n¼112) of respondents indicated they have an ultrasound machine available in their emergency department, and 78% indicated they use UGCVC. Seventy-seven percent (n ¼ 90) agreed with the statement, ‘‘Ultrasound guidance is the preferred method for central venous catheter placement in the emergencydepartment.’’ However, 23% of respondents stated they have received no specific training in UGCVC. Led by university students, the movement challenged the longstanding free-market orientation of educational policies in Chile, demanding a more active role for the State in the regulation and supply of education. In this article, we study the main educational and social factors that explain the emergence of the movement. We draw upon social movement theory as an analytical framework and use newspaper articles as basic sources of data. Our research suggests that the simultaneous expansion and privatization of the Chilean education system provided students not only with mobilizing grievances (e.g., disparity in access and quality) but also with capabilities and resources (e.g., critical awareness and higher aspirations) to advance political mobilization. We also find that student organizations created effective frames to take advantage of the windows of opportunity opened in Chilean democracy. This article reports the NGO leaders’ perceptions of the goals, strategies, and challenges in their citizenship endeavor, and analyzes their educational activities in theoretical and cultural contexts.
