            "abstract": "In this critical review of the literature, I interrogate the assumptions underlying STEM workforce studies as it pertains to gender, race, class, and citizenship. First, I provide a brief overview of the pipeline model’s history and critiques. Next, I look at the contemporary use of the model in STEM workforce studies, focusing on the ways in which recruitment and retention, scientific work, and identity are represented, measured, and understood. I argue throughout that the pipeline model has a limited view of retention that is based upon socially constructed ideas about what constitutes “valid” scientific and engineering work and who counts as “real” scientists and engineers.",

            "abstract": "The topic of this paper is the self-imposed limits of Library and Information Science discourse and its institutional discipline. In particular, this paper discusses the disciplinary limits that the field places upon itself, its phobia regarding critical theory and interdisciplinary work (outside of computer science), and why public information, such as 'the news,' is not seen as part of our domain of inquiry. It also engages how persons are understood and constructed as 'information seeking' subjects in this field, including LIS students and researchers. Finally are questions of the overarching disciplining of students and researchers toward 'positive' research in the field, a research that is, in part, often founded upon very shaky 'foundational' theoretical models. Arguably, these questions are linked in the construction of an 'informationalized,' rather docile and uninteresting, political subject, both within and outside of information research in the university, both within and outside of information professionalism, and in the public at large, which should all now be educated to be \"information professionals\" in a critical manner. All of this is more striking given the amount of verbiage in the past twenty years or so about the presence and the importance of 'the information age.' These questions are specific to Library and Information Science, but they also extend out to information science more generally understood and to questions about the formation of subjectivity in the contemporary university and in U.S. politics.  Issues regarding method and critique are central in this paper.",

            "title": "Benthic Assemblage Variability in the Upper San Francisco Estuary: A 27-Year Retrospective",

            "abstract": "We used multivariate methods to explore changes in benthic assemblage structure over 27 years (1977–2003) at four monitoring stations located along a salinity gradient in the upper San Francisco Estuary. Changes in benthic assemblage composition were assessed relative to hydrologic variability and to the presence of the high-impact invader \nCorbula amurensis \nin the estuary. We also explored the composition of benthic assemblages during a recent collapse of several pelagic populations in the upper estuary. Our results show that the \nCorbula\n invasion had both direct and indirect effects on the benthos in the estuary, causing significant changes in assemblage structure. We found no unprecedented patterns of benthic assemblage composition during the period of the Pelagic Organism Decline (2000–2003) in the upper estuary. Hydrologic variability was associated with significant changes in benthic assemblage composition at all locations. Benthic assemblage composition was more sensitive to mean annual salinity than other local physical conditions. That is, benthic assemblages were not geographically static, but shifted with salinity, moving down-estuary in years with high delta outflow, and up-estuary during years with low delta outflow, without strong fidelity to physical habitat attributes such as substrate composition or location in embayment vs. channel habitat. Organism abundance and species richness showed a bi-modal distribution along the salinity gradient, with lowest abundance and richness in the 5 to 8 psu range. We conclude that the continuity of benthic assemblages and community metrics along the salinity gradient is a powerful and necessary context for understanding historical variability in assemblage composition at geographically static monitoring stations.",

            "title": "Mercury-Contaminated Hydraulic Mining Debris in San Francisco Bay",

            "abstract": "The hydraulic gold-mining process used during the California Gold Rush and in many developing countries today contributes enormous amounts of sediment to rivers and streams. Commonly, accompanying this sediment are contaminants such as elemental mercury and cyanide used in the gold extraction process. We show that some of the mercury-contaminated sediment created by hydraulic gold mining in the Sierra Nevada, between 1852 and 1884, ended up over 250 kilometers (km) away in San Francisco Bay; an example of the far-reaching extent of contamination from such activities. A combination of radionuclide dating, bathymetric reconstruction, and geochemical tracers were used to distinguish the hydraulic mining sediment from sediment deposited in the bay before hydraulic mining started (pre-Gold Rush sediment) and sediment deposited after hydraulic mining stopped (modern sediment). Three San Francisco Bay cores were studied as well as source material from the abandoned hydraulic gold mines and river sediment between the mines and bay. Isotopic and geochemical compositions of the core sediments show a geochemical shift in sediment deposited during the time of hydraulic mining. The geochemical shift is characterized by a decrease in εNd, total organic carbon (TOC), Sr and Ca concentrations, Ca/Sr, and Ni/Zr; and, an increase in 87Sr/86Sr, Al/Ca, Hg concentrations, and quartz/plagioclase. This shift is in the direction of the geochemical signature of sediments from rivers and gold mines in hydraulic mining areas. Mixing calculations using Nd isotopes and concentrations estimate that the hydraulic mining debris comprises up to 56% of the sediment in core sediments deposited during the time of hydraulic mining. The surface sediment of cores taken in 1990 were found to contain up to 43% hydraulic mining debris, reflecting a continuing remobilization and redistribution of the debris within the bay and transport from the watershed. Mercury concentrations in pre-Gold Rush sediment range between 0.03 and 0.08 μg g-1. In core sediments that have characteristics of the gold deposits and were deposited during the time of hydraulic mining, mercury concentrations can be up to 0.45 μg/g. Modern sediment (post-1952 deposition) contains mercury concentrations up to 0.79 μg/g and is likely a mix of hydraulic mining mercury and mercury introduced from other sources.",

            "title": "Potential Inundation Due to Rising Sea Levels in the San Francisco Bay Region",

            "abstract": "An increase in the rate of sea level rise is one of the primary impacts of projected global climate change. To assess potential inundation associated with a continued acceleration of sea level rise, the highest resolution elevation data available were assembled from various sources and mosaicked to cover the land surfaces of the San Francisco Bay region. Next, to quantify extreme water levels throughout the bay, a hydrodynamic model of the San Francisco Estuary was driven by a projection of hourly water levels at the Presidio. This projection was based on a combination of climate model outputs, an empirical model, and observations, and incorporates astronomical, storm surge, El Niño, and long-term sea level rise influences.\n \nBased on the resulting data, maps of areas vulnerable to inundation were produced, corresponding to specific amounts of sea level rise and recurrence intervals, including tidal datums. These maps portray areas where inundation will likely be an increasing concern. In the North Bay, wetlands and some developed fill areas are at risk. In Central and South bays, a key feature is the landward periphery of developed areas that would be newly vulnerable to inundation. Nearly all municipalities adjacent to South Bay face this risk to some degree. For the bay as a whole, as early as mid-century under this scenario, the one-year peak event nearly equals the 100-year peak event in 2000.             "abstract": "In the handful of existing comparative false belief studies, chimpanzees have consistently failed tests that 5- to 6-year-old children have passed. However, those tests were either explicitly cooperative communicativeor competitive, both of which are problematic for different reasons. We therefore devised a new change-of-contents false belief test for children and chimpanzees that did not include these problematic elements. Nevertheless, chimpanzees showed no evidence of understanding false beliefs (consistent with past research, however, children showed a clear improvement in test performance from 3.5 to 4.5 years of age). Our results suggest that the cooperative-communicative or competitive nature of previous false belief tests was not solely responsible for chimpanzees’ failure. That chimpanzees have now also failed a more socially neutral test supports the conclusion that chimpanzees may simply not recognize false belief states in others. Additionally, our test departs from a near exclusive reliance on the change-of-location paradigm in false belief research. It therefore expands the repertoire of methods available for testing false belief understanding with minimally verbal and nonverbal procedures.",

            "abstract": "Pointing is a conventional communicative gesture used by humans to direct others’ attention to an environmental feature. Several researchers have argued that pointing becomes so ingrained for humans from a young age that children often have difficulty interpreting the gesture in a novel way. Recent research suggests domestic dogs are also sensitive to human gestures (including points) and proficient in recognizing and acting on humans’ visual attention. We explored the role of pointing indogs’ choice behavior and whether dogs, like human children, have difficulty interpreting the gesture novelly. In Experiment 1, we explored whether dogs would differentially follow a static human point when it was administered by a familiar or unfamiliar individual and that individual indicated or failed to indicate the correct location of a food reward. The results indicated dogs chose the container specified by the demonstrators’ point in the honest and dishonest condition. Demonstrator familiarity did not alter performance. In Experiment 2, we compared dogs’ propensity to follow a static point versus other cues (momentary point, standing location) when the cue never indicated the correct location of a food reward, which was either visible or hidden during choice. The results suggested dogs did not inhibit their approach to a location indicated by a deceptive static point even when thelocation of a reward was visibly available during choice. However, dogs used a deceptive momentary point or standing location to locate food in both visible and hidden trials. In Experiment 3, we explored if dogs could overcome their tendency to follow a deceptive static point. These results indicated dogs learned to inhibit their approach to a deceptive static point when the reward was visible during choice. However, when information about the reward’s location was later hidden, dogs reverted to following the demonstrator’s static point.",

            "abstract": "We examined whether aversive conditioning of a previously established conditioned inhibitor (A) for sucrose solution (X) affects subsequent consumption and aversive conditioning of X. Experiment 1 established an appetitive conditioned inhibition procedure in which odor A became a conditioned inhibitor for X. In Phase 1 of Experiments 2 and 3, subjects received either inhibitory (Group INH: AB/BX/C) or excitatory conditioning of A (Group EXC: ABX/B/C), or a control treatment (Group CONT: AB/B/CX). In Phase 2, A was paired with an injection of lithium chloride (LiCl) and consumption of X was measured. X was paired with LiCl in Phase 3, and tested in extinction. After a moderate amount of Phase 1 treatment in Experiment 2, animals in Group EXC showed a reduction in consumption of X after A-LiCl pairings, while those in both Groups EXC and INH rapidly acquired an aversion to X during X-LiCl conditioning. However, when extended Phase 1 treatment was given in Experiment 3, animals in Group INH tended to acquire the aversion to X at a slower rate than those in Group CONT. Animals in Group EXC did not show any superiority in acquisition of the X aversion. The results are discussed in terms of mediation processes by event representations.",

            "abstract": "Macaques are the most geographically widespread and behaviorally diverse primate genus, and although macaque species share the same, basic social structure, they display broad inter specificvariation in patterns of adult social behavior. Based on these patterns, macaque species have been arranged along a 4-grade scale for social style. At one end of the scale, there are grade 1 species (e.g., Japanese macaques) that have highly hierarchical and despotic social systems, and at the other end, grade 4 species (e.g., Tonkean macaques) that have more relaxed and egalitarian social systems. We predicted that a species from the more despotic end of the spectrum should have more competitive play fights and that a species from the egalitarian end, more cooperative ones. A detailed analysis of videotaped sequences of juvenile play fighting in Tonkean and Japanese macaques was used to characterize the targets and tactics of attack and defense. Even though the two species have a similar behavioral repertoire, there are significant differences in how that repertoire is used and these differences are consistent with one species having more competitive interactions than the other. Contrasting multi-animal play fights versus pairs showed that the more cooperative style of the Tonkean macaques is further exaggerated. The results suggest that differences in styles of attack anddefense in play fighting may be influenced by differences in the species’ social systems.",

            "abstract": "In light of the growing importance of identity work in second language acquisition (e.g., Block, 2006a, b) in Second Language Acquisition (SLA) as well as calls for SLA and World Englishes (WE) scholars (e.g., Y. Kachru, 2005) to work together, I examine how identity has been conceptualized in research on the global use of English. While such research finds its roots in the WE paradigm (e.g, B. Kachru, 2005), it has undergone contestation in recent years. Such contestation has emerged as a result of two new conceptualizations of English: English as a lingua franca (e.g., Jenkins, 2007; Seidlhofer, 2006) and a postmodern approach to English (e.g., Canagarajah, 2006; Pennycook, 2007, 2010), which views it in hybrid and fluid terms. This paper explores how identity has been embodied in the literature on the global use of English with a view to analyzing how future SLA research related to identity should take shape in the face of changes brought about by globalization.",

Most published materials, usually based on an author’s personal preferences or current methodological recommendations, emphasize the importance of presenting one’s self well and successfully establishing certain expectations for student behavior from the beginning of an academic term. However, little is known regarding what SL teachers actually say and do on the first day of class or how students perceive this crucial first meeting. Grounded in empirical data including classroom visits, teacher interviews, and student observations, the present qualitative study explores five university-level SL teachers’ approaches to the first day of class. Specifically, this study analyzes these teachers’ explicit and implicit communication of expectations regarding classroom rules and regulations (Johnston, Juhász, Marken & Ruiz, 1998) on the first day. Recent research on the morality of teaching (Jackson, Boostrom & Hansen, 1993; Johnston 2003) provides the framework for the data analysis. In the present study, teachers’ words and actions revealed characteristics of their moral agency, exposed actual teaching practices, and have important implications for SL pedagogy that are also relevant to teaching beliefs and practices in other disciplines.",

            "abstract": "The authors compare the frequency of negotiations for meaning in a natural spoken corpus to a variety of cohesive devices. The study demonstrates that a lack of cohesive devices in non-native speaker (NNS) discourse correlates to negotiations for meaning. The data comes from a year-long longitudinal study of six beginning NNSs and comprised 99 transcripts. The transcripts were coded for negotiations for meaning. Regression analyses suggested that causal cohesion and semantic co-referentiality were significantly related to the frequency of negotiations for meaning. Additionally, NNS discourse demonstrates a significant decrease in frequency of negotiations for meaning as a function of time. Taken together, these results suggest that negotiations for meaning are related to a lack of cohesive devices in NNS speech.",

            "abstract": "Most studies of language maintenance and loss in the United States have concentrated on contact between English and one specific heritage or minority language. The present study examines the experience of a Spanish-Italian immigrant family and the factors they identify as key in shaping their patterns of language use through three generations. The family on which the present analysis is centered is unique: the participants are European, they do not live in a Spanish-speaking community, and because members of the first generation immigrated in the mid 1950’s, their views of acculturation are different from those of more recent immigrants. This family’s story provides insight into the immigrant experience and highlights the potential role of heritage and ethnic pride as a means of motivating students to pursue the study of foreign languages.",

            "abstract": "This study lays the foundation for reasons and ways to investigate the biased treatment of non-native characters in model dialogues in current English as a Foreign/Second Language textbooks. The literature review shows that although a plethora of studies have been conducted on gender bias in textbooks, speaker bias, or labeled nativism here, has been largely ignored. This research addresses this neglect by systematically applying parts of two frameworks previously used in analyzing textbooks for gender bias to four current EFL textbooks. The resulting data is quantitative in nature with some necessary description and qualification and shows that only one text avoids bias against non-native speakers. In the other texts, speaker bias is exhibited by non-native speakers being segregated or being only allowed to interact with a native speaker. In addition, non-native speakers are limited to non-expert roles in two texts. Based on these results, suggestions for further research are offered.",

            "abstract": "Objectives: Heart failure is one of the leading causes of death in the U.S. The incorporation of B-type natriuretic peptide (BNP) measurements when triaging patients presenting with shortness of breath has improved the diagnostic and prognostic ability of physicians. Currently, there are no point-of-care systems for quantifying BNP that can be used without sacrificing accuracy. We compared the analytical performance of the Abbott i-STAT analyzer, a handheld point-of-care system for measuring BNP, with the lab-based system, the Abbott ARCHITECT.\n\n\nMethods: One-hundred fifty samples were collected from three clinical settings: 41 from the Emergency Department, 58 from the inpatient wards, and 51 from heart failure outpatient clinics. Linear regression and bias difference analyses were run to evaluate the accuracy of the i-STAT. Correlation between the i-STAT and Architect BNP values were made with values of BNP.\n\n\nResults: The correlation coefficient was r=0.977 (N=150, p<.0001). The average bias was significant (-36) and there were concentration-dependent differences at higher BNP values. Precision of the i-STAT was poor compared to the lab-based platform.\n\n\nConclusion: Although the precision of the i-STAT was poor, there was good clinical agreement between the i-STAT and the lab-based platform.             "title": "Dedicated Shift Wrap-up Time Does Not Improve Resident Sign-out Volume or Efficiency",

            "abstract": "Objectives: Sign-out (SO) is a challenge to the emergency physician. Some training programs have instituted overlapping 9-hour shifts. The residents see patients for eight hours, and have one hour of wrap-up time. This hour helps them complete patient care, leaving fewer patients to sign-out. We examined whether this strategy impacts SO burden.\n\n\nMethods: This is a retrospective review of patients evaluated by emergency medicine (EM) residents working 9-hour (eight hours of patient care, one hour wrap-up time) and 12-hour shifts (12 hours patient care, no reserved time for wrap-up). Data were collected by reviewing the clinical tracker. A patient was assigned to the resident who initiated care and dictated the chart. SO was defined as any patient in the ED without disposition at change of shift. Patient turn-around-time (TAT) was also recorded.\n\n\nResults: One-hundred sixty-one postgraduate-year-one resident (PGY1), 264 postgraduate-year-two resident (PGY2), and 193 postgraduate-year-three resident (PGY3) shifts were included. PGY1s signed out 1.9 patients per 12-hour shift. PGY2s signed out 2.3 patients on 12-hour shifts and 1.8 patients on 9-hour shifts. PGY3s signed out 2.1 patients on 12-hour shifts and 2.0 patients on 9-hour shifts. When we controlled for patients seen per hour, SO burden was constant by class regardless of shift length, with PGY2s signing out 18% of patients seen compared to 15% for PGY3s. PGY1s signed out 18% of patients seen. TAT for patients seen by PGY1s and PGY2s was similar, at 189 and 187 minutes, respectively. TAT for patients seen by PGY3s was significantly less at 175 minutes.\n\n\nConclusion: The additional hour devoted to wrapping up patients in the ED had no affect on SO burden. The SO burden represented a fixed percentage of the total number of patients seen by the residents. PGY3s sign-out a smaller percentage of patients seen compared to other classes, and have faster TATs. [West J Emerg Med. 2010; 11(1):35-39].",

            "abstract": "Objective: Glycemic control in the critically ill intensive care unit (ICU) patient has been shown to improve morbidity and mortality. We sought to investigate the effect of early glycemic control in critically ill emergency department (ED) patients in a small pilot trial.\n\n\nMethods: Adult non-trauma, non-pregnant ED patients presenting to a university tertiary referral center and identified as critically ill were eligible for enrollment on a convenience basis. Critical illness was determined upon assignment for ICU admission. Patients were randomized to either ED standard care or glycemic control. Glycemic control involved use of an insulin drip to maintain blood glucose levels between 80-140 mg/dL. Glycemic control continued until ED discharge. Standard patients were managed at ED attending physician discretion. We assessed severity of illness by calculation of APACHE II score. The primary endpoint was in-hospital mortality. Secondary endpoints included vasopressor requirement, hospital length of stay, and mechanical ventilation requirement.\n\n\nResults: Fifty patients were randomized, 24 to the glycemic group and 26 to the standard care cohort. Four of the 24 patients (17%) in the treatment arm did not receive insulin despite protocol requirements. While receiving insulin, three of 24 patients (13%) had an episode of hypoglycemia. By chance, the patients in the treatment group had a trend toward higher acuity by APACHE II scores. Patient mortality and morbidity were similar despite the acuity difference.\n\n\nConclusion: There was no difference in morbidity and mortality between the two groups. The benefit of glycemic control may be subject to source of illness and to degree of glycemic control, or have no effect. Such questions bear future investigation.             "abstract": "Objective: To determine whether a medical student emergency ultrasound clerkship has an effect on the number of patients undergoing ultrasonography and the number of total scans in the emergency department.\n\n\nMethods: We conducted a prospective, single-blinded study of scanning by emergency medicine residents and attendings with and without medical students. Rotating ultrasound medical students were assigned to work equally on all days of the week. We collected the number of patients scanned and the number of scans, as well as participation of resident and faculty.\n\n\nResults: In seven months 2,186 scans were done on the 109 days with students and 707 scans on the 72 days without them. Data on 22 days was not recorded. A median of 13 patients per day were scanned with medical students (CI 12-15) versus seven (CI 6-9) when not. In addition, the median number of scans was 18 per day with medical students (CI 16-20) versus eight (CI 6-10) without them.\n\n\nConclusion: There were significantly more patients scanned and scans done when ultrasound medical students were present.             "title": "Emergency Department Chief Complaint and Diagnosis Data to Detect Influenza-Like Illness with an  Electronic Medical Record",

            "abstract": "Background: The purpose of syndromic surveillance is early detection of a disease outbreak. Such systems rely on the earliest data, usually chief complaint. The growing use of electronic medical records (EMR) raises the possibility that other data, such as emergency department (ED) diagnosis, may provide more specific information without significant delay, and might be more effective in detecting outbreaks if mechanisms are in place to monitor and report these data.\n\n\nObjective: The purpose of this study is to characterize the added value of the primary ICD-9 diagnosis assigned at the time of ED disposition compared to the chief complaint for patients with influenza-like illness (ILI).\n\n\nMethods: The study was a retrospective analysis of the EMR of a single urban, academic ED with an annual census of over 60, 000 patients per year from June 2005 through May 2006. We evaluate the objective in two ways. First, we characterize the proportion of patients whose ED diagnosis is inconsistent with their chief complaint and the variation by complaint. Second, by comparing time series and applying syndromic detection algorithms, we determine which complaints and diagnoses are the best indicators for the start of the influenza season when compared to the Centers for Disease Control regional data for Influenza-Like Illness for the 2005 to 2006 influenza season using three syndromic surveillance algorithms: univariate cumulative sum (CUSUM), exponentially weighted CUSUM, and multivariate CUSUM.\n\n\nResults: In the first analysis, 29% of patients had a different diagnosis at the time of disposition than suggested by their chief complaint. In the second analysis, complaints and diagnoses consistent with pneumonia, viral illness and upper respiratory infection were together found to be good indicators of the start of the influenza season based on temporal comparison with regional data. In all examples, the diagnosis data outperformed the chief-complaint data.\n\n\nConclusion: Both analyses suggest the ED diagnosis contains useful information for detection of ILI. Where an EMR is available, the short time lag between complaint and diagnosis may be a price worth paying for additional information despite the brief potential delay in detection, especially considering that detection usually occurs over days rather than hours.             "title": "Emergency Department Crowding: Factors Influencing Flow",

            "abstract": "Background: The objective of this study was to evaluate those factors, both intrinsic and extrinsic to the emergency department (ED) that influence two specific components of throughput: “door-to-doctor” time and dwell time.\n\n\nMethods: We used a prospective observational study design to determine the variables that played a significant role in determining ED flow. All adult patients seen or waiting to be seen in the ED were observed at 8pm (Monday-Friday) during a three-month period. Variables measured included daily ED volume, patient acuity, staffing, ED occupancy, daily admissions, ED boarder volume, hospital volume, and intensive care unit volume. Both log-rank tests and time-to-wait (survival) proportional-hazard regression models were fitted to determine which variables were most significant in predicting “door-to-doctor” and dwell times, with full account of the censoring for some patients.\n\n\nResults: We captured 1,543 patients during our study period, representing 27% of total daily volume. The ED operated at an average of 85% capacity (61-102%) with an average of 27% boarding. Median “door-to-doctor” time was 1.8 hours, with the biggest influence being triage category, day of the week, and ED occupancy. Median dwell time was 5.5 hours with similar variable influences.\n\n\nConclusion: The largest contributors to decreased patient flow through the ED at our institution were triage category, ED occupancy, and day of the week. Although the statistically significant factors influencing patient throughput at our institution involve problems with inflow, an increase in ED occupancy could be due to substantial outflow obstruction and may indicate the necessity for increased capacity both within the ED and hospital.             "title": "Emergency Department Septic Screening in Respiratory Syncytial Virus (RSV) and Non-RSV Bronchiolitis",

            "abstract": "Objective: To identify factors associated with culture-proven serious bacterial infection (SBI) and positive emergency department septic screening (EDSS) tests in children with bronchiolitis and to identify factors associated with the performance of EDSS.\n\n\nMethods: We reviewed an existing study database of patients with bronchiolitis. We defined a positive EDSS as urine with ≥10 WBC per high power field or cerebrospinal fluid (CSF) with ≥10 WBC per high power field (>25 WBC in neonates), or if organisms were identified on gram stain. We defined SBI as significant growth of an accepted pathogen in blood, urine or CSF. Our composite endpoint was positive if either of these was positive. The decision to perform testing was modeled using modified Poisson regression; the presence of the combined outcome was modeled using logistic regression modified for rare events.\n\n\nResults: We studied 640 children. Testing was performed in 199/640 (31.1%). These tended to be younger than two months RR 2.69 (95% CI 2.11, 3.44), febrile RR 2.01 (95% CI 1.58, 2.55), more dehydrated RR 1.50 (95% CI 1.28, 1.75) and had more severe chest wall retractions RR 1.54 (95% CI 1.22, 1.94). Only 11/640(1.7%) had a positive EDSS or SBI. Younger age (OR 0.67 per month; 95% CI 0.45, 0.99) and a negative RSV antigen test (OR 6.22; 95% CI 1.30, 29.85) were associated with the composite endpoint.\n\n\nConclusion: Testing was more likely to be performed in children younger than two months of age, and in those who were febrile, dehydrated, and had more severe chest wall retractions. A positive EDSS or SBI was rare occurring in younger infants with non-RSV bronchiolitis.  [West J Emerg Med. 2010; 11(1):60-67].",

            "title": "Eschmann Introducer Through Laryngeal Mask Airway: A Cadaveric Trial of An Alternate Means of Rescue Intubation",

            "abstract": "Study Objective: Laryngeal mask airways (LMAs) are often used as airway rescue devices where laryngoscopy is difficult. The LMA does not protect the airway and is preferably replaced with a cuffed endotracheal tube. There are reports of cases where an Eschmann tracheal tube introducer (ETTI) was successfully used to bridge between a standard LMA and an endotracheal tube. This project was designed to determine whether an Eschmann stylet can reliably be passed through an LMA into the trachea as a means of rescue intubation.\n\n\nMethods: Nineteen emergency medicine residents and attending physicians, who were participants in a cadaveric airway course, placed and inflated a size 4 LMA (The Laryngeal Mask Company Ltd., San Diego, CA) on each of six unembalmed human cadavers in the usual fashion. They then attempted to pass a lubricated, 15 Fr, reusable, coude-tipped ETTI (Portex, Smiths Medical, Keene, NH)) through the airspace/handle of the inflated LMA. The LMA was then deflated and removed while the ETTI was held in place. Investigators then determined the location of the ETTI by laryngoscopy.\n\n\nResults: Of 114 attempts at the rescue procedure, 59 resulted in placement of the bougie into the trachea, yielding an overall success rate of 52% (95% CI 48%-56%). There were no significant differences in performance based on level of training of residents or years of experience of attending physicians.\n\n\nConclusions: While not a primary difficult airway option, the use of a ETTI as a bridge device between LMA and endotracheal tube was successful about 50% of the time.             "abstract": "We present a case of fatal rebound hyperkalemia in a patient with thyrotoxic periodic paralysis (TPP) treated with potassium supplementation. Although TPP is a rare hyperthyroidism-related endocrine disorder seen predominantly in men of Asian origin, the diagnosis should be considered in patients of non-Asian origins presenting with hypokalemia, muscle weakness or acute paralysis. The condition may present as a life threatening emergency and unfamiliarity with the disease could result in a fatal outcome. Immediate therapy with potassium chloride supplementation may foster a rapid recovery of muscle strength and prevent cardiac arrhythmias secondary to hypokalemia, but with a risk of rebound hyperkalemia.             "abstract": "Objective: To determine the incidence and frequency of follow-up instructions for incidental findings on computed tomography (CT) scanning of the abdomen and pelvis in trauma patients.\n\n\nMethods: We performed a retrospective chart review of all adult patients triaged to the trauma service at a Level I trauma center between July 1, 2003 and June 30, 2004. Included patients were 16 years of age and older who underwent abdomen and pelvic CT scans as part of their primary evaluation. We excluded patients under the age of 16 years, patients unable to complete radiographic studies due to deterioration in condition, patients with missing CT scan reports, and transferred patients who had CT scans done at outside facilities.\n\n\nResults: A total of 1,633 patients presented to the trauma service during the study period; 922 patients met inclusion criteria. Of these, 392 had incidental findings noted on the formal radiology report. Twenty patients with incidental findings either received additional workup during their hospital admission for their trauma injuries or were notified of the findings on discharge. Nine died prior to discharge. One hundred twenty-two patients with incidental findings had those findings noted in the history and physical or discharge summary with no documentation of follow-up. There was no documentation of any incidental findings in the electronic record for the majority of patients (242) with incidental findings.\n\n\nConclusion: The majority of incidental findings discovered on abdomen and pelvic CT scanning of trauma patients are not documented; therefore, many patients may not receive the appropriate recommended follow up.             "abstract": "Necrotizing fasciitis (NF) is a rare and lethal soft tissue infection that requires urgent surgical intervention. It is most often found in the extremities occurring with precipitating trauma or in immunocompromised states. Signs and symptoms are often vague or missing making early diagnosis very difficult. Our patient presented with flank pain and altered mental status but no known precipitating factors. Computed Tomography showed gas within and around the right paraspinous muscle suspicious for NF. Given NF’s high lethality, early suspicion by emergency physicians of NF in patients with soft tissue infections or with systemic findings of unknown etiology is necessary.             "abstract": "Injury is the leading cause of death and disability among the U.S. population aged 1 to 44 years. In 2006 more than 179,000 fatalities were attributed to injury. Despite increasing awareness of the global epidemic of injury and violence, a considerable gap remains between advances in injury-prevention research and prevention knowledge that is taught to medical students. This article discusses the growing need for U.S medical schools to train future physicians in the fundamentals of injury prevention and control. Teaching medical students to implement injury prevention in their future practice should help reduce injury morbidity and mortality. Deliberate efforts should be made to integrate injury-prevention education into existing curriculum. Key resources are available to do this. Emergency physicians can be essential advocates in establishing injury prevention training because of their clinical expertise in treating injury. Increasing the number of physicians with injury- and violence- prevention knowledge and skills is ultimately an important strategy to reduce the national and global burden of injury.             "abstract": "Over the past decade, capsule endoscopy has become the accepted modality for small bowel imaging in the United States. It is very helpful in making the diagnosis of Crohn’s disease; however, this patient population is also at an increased risk of small bowel obstruction secondary to capsule impaction. We present the case of a 60-year-old female with undiagnosed Crohn’s disease who presented to the emergency department with small bowel obstruction after capsule ingestion. She was successfully disimpacted with diatrizoate upper gastrointestinal (GI) series with small bowel follow-through and intravenous steroids.             "abstract": "This case report describes a patient with a subacute right-sided tension hemopneumothorax following an occult stab. The patient’s electrocardiogram (ECG), performed as part of a standardized triage process, demonstrated significant abnormalities that misguided initial resuscitation, but resolved following evacuation of the tension hemopneumothorax. Tension pneumothorax is typically regarded as an immediately life-threatening condition that requires emergent management with needle or tube thoracostomy. However, we believe that subacute tension pneumothorax may be a rarely observed clinical phenomenon and may lead to unique ECG findings. We believe that the ECG changes we observed provided an early clue to the eventual diagnosis of a subacute tension pneumothorax and have not been previously described in this setting. .             "title": "The Physiologic Effects of Multiple Simultaneous Electronic Control Device Discharges",

            "abstract": "Objectives: Law enforcement and military personnel use electronic control devices to control non-compliant and actively resistive subjects. The TASER® Shockwave is a new electronic control device designed specifically as an area denial device capable of delivering multiple simultaneous discharges. This is the first study to examine the effects of multiple simultaneous device discharges in humans.\n\n\nMethods: Volunteers were exposed to multiple (two to three), simultaneous 5-second discharges from the Shockwave device to the chest, back, chest to abdomen, or thighs. Blood was analyzed before and after discharge for pH, lactate, potassium, creatine kinase (CK), and troponin. Continuous spirometry was performed before, during, and after the discharge. In addition, electrocardiograms (ECGs) before and after discharge were recorded, and echocardiography was used to determine the rhythm during discharge.\n\n\nResults: Small elevations of lactate occurred. Moderate increases in CK at 24 hours occurred and appeared to be related to the number of simultaneous discharges. There was a trend to a decrease in minute ventilation in the volunteers exposed to two simultaneous discharges, but it did not reach statistical significance. ECG changes only reflected an increase in vagal tone, and there was no evidence of capture by echocardiography. Five-second, simultaneous, multiple exposures to the TASER Shockwave device were reasonably tolerated by our human volunteers.\n\n\nConclusion: Our study suggests that this device may have a reasonable risk/benefit ratio when used to protect an area from a threat.             "title": "Food For Thought: The Social Impact of Community Gardens in the Greater Cleveland Area",

            "abstract": "While the benefits of healthy eating and greenspace development have been well documented, the social impact of urban and community gardens remain less studied.  This paper explores the social and cultural effects of urban gardening in the greater Cleveland area.  Gardening is shown to have a multitude of motivating factors, including economic, environmental, political, social, and nutritional.  While analyzing the impact that gardens have on community building, identity, and food security, some authors claim that the gardeners themselves are preoccupied with the economic impact of their actions.  Perversely, this leads readers to the conclusion that poor people or people of color are only interested in gardening for its dollar value. Following this argument, more affluent gardeners have the security to ignore the economic impact and focus only on furthering an environmentalist agenda.  Such authors presume that utilitarian function and environmentalist ideology are mutually exclusive, but my own fieldwork showed that many gardeners actively combine these ideas.  This paper intends to convey the complexity of use, function, and intent in these communal spaces, filling an existing gap in our understanding of their social impact.",

            "abstract": "The concept of sustainable development emerged to offer a fresh perspective on the traditional developmental paradigm and to attempt to provide global consensus as to how to achieve sound environmental goals in a world characterized by economic disparities.  In Trinidad and Tobago, the new environmental regime, which was introduced in 1995, had at its cornerstone a two-pronged approach to environmental protection.  The first was the introduction of the requirement for special approval of a list of designated activities covering the major economic sectors of Trinidad and Tobago.  This approval process contemplated resolution of major environmental impacts associated with specific economic activities prior to any approval being granted for such activities.  The second prong was a permitting system that would see a gradual reduction in the output of pollutants from existing industry.  The aim, therefore, was the stabilisation of the output of pollutants by controlling the actions of new economic players and the gradual reduction of the existing pollution inventory through the permitting system.  This paper will focus specifically on the use of the Environmental Impact Assessment (“EIA”) process for the making of developmental decisions with a brief examination of how the permitting process works in Trinidad and Tobago.",
